Pharmacodynamics and pharmacokinetics

The active ingredient is beclomethasone . The aerosol is effectively used in the treatment of bronchial asthma (basic therapy).

Aldecine has antiallergic and moderate anti-inflammatory effects. Beclomethasone enhances the production of the A2 phospholipase inhibitor - annexin (lipocortin), inhibits the production of inflammatory mediators, reduces the release of arachidonic acid , and has an inhibitory effect on the synthesis of arachidonic acid metabolic products - prostaglandins and endoperoxides.

The anti-inflammatory effect is due to inhibition of the formation of chemotaxis , the formation of a slow allergic reaction slows down.

Belomethasone improves mucociliary transport , reduces the number of mast cells in the bronchial mucosa, reduces bronchial hyperreactivity, and reduces the secretion of mucus produced by the bronchial glands.

Aldecine has a positive effect on functional indicators of external respiration. The drug allows you to restore the patient's response to the effects of bronchodilators, which significantly reduces the frequency of their use.

The drug increases the number of active beta-adrenergic receptors and does not have a resorptive effect when used in inhalation.

The use of the drug Aldecin in therapeutic doses does not lead to the development of systemic side effects that occur when using glucocorticosteroids. The therapeutic effect develops gradually; the drug does not relieve bronchospasm .

How to take Nasonex

The drug is intended for intranasal administration of the suspension contained in the bottle through a dosing nozzle. The dose of the drug sprayed into the nasal cavity is determined by the doctor depending on the severity of the disease.

Before using the spray for the first time, it is “calibrated” by pressing the applicator 6-7 times. This action allows you to establish the delivery of the drug into the nasal cavity in a therapeutic dose of 100 mg.

Taking Nasonex

Before each use, the bottle must be shaken vigorously.

For direct use, the end of the dispensing nozzle must be inserted into the nasal passage and pressed on the applicator (1 press = 1 spray). The head and bottle with the drug must be kept strictly vertical.

If Nasonex has not been used for 14 days or more, it is necessary to re-calibrate by pressing the applicator 2 times. It is also important to clean the dispensing nozzle regularly to prevent it from malfunctioning.

Contraindications

Aldecine is contraindicated in non-asthmatic bronchitis, acute bronchospasm, beclomethasone intolerance, and status asthmaticus.

Contraindications also include: tuberculosis, nosebleeds, age under 6 years.

For glaucoma , liver cirrhosis, breastfeeding , osteoporosis , hypothyroidism , pregnancy , systemic diseases (viral, parasitic, fungal, bacterial infections) prescribe with caution.

Aerosol for inhalation Aldecin

Instructions for medical use of the drug

Description of pharmacological action

Beclomethasone dipropionate - GCS, has anti-inflammatory, antiallergic and immunosuppressive effects.

Indications for use

Allergic rhinitis (seasonal or year-round); Vasomotor rhinitis; Nasal polyposis (as part of combination therapy).

Release form

aerosol for inhalation dosed 50 mcg/dose; aerosol can (can) with nozzles for nasal administration and spraying in the mouth, box (box) 1.

Pharmacodynamics

Beclomethasone dipropionate - GCS, has anti-inflammatory, antiallergic and immunosuppressive effects. Inhibits the release of inflammatory mediators from mast cells, increases the production of lipomodulin, which is an inhibitor of phospholipase A, reduces the release of arachidonic acid, inhibits the synthesis of arachidonic acid metabolic products - cyclic endoperoxides, PGs. Reduces swelling and secretion of glands of the nasal mucosa, improves mucociliary transport. It has virtually no mineralocorticoid activity and no resorptive effect after inhalation administration.

Pharmacokinetics

Absorption is low; when administered by inhalation in recommended doses, it does not have clinically significant systemic activity. After inhalation, it is possible to swallow part of the administered dose with saliva. Most of the drug that enters the gastrointestinal tract is inactivated by the “first pass” effect through the liver. Plasma protein binding - 87%. T1/2 - 15h. The main part of the drug (35–76%) is excreted within 96 hours in the feces, mainly in the form of polar metabolites, 10–15% by the kidneys.

Use during pregnancy

Aldecine can be used in pregnant and lactating women only if the possible benefit of treatment for the woman justifies the potential risk to the fetus or newborn. Newborn children of women who received corticosteroids during pregnancy should be examined to identify a possible decrease in adrenal function.

Contraindications for use

Hypersensitivity to any of the components of the drug; tuberculosis (including latent) of the respiratory system; systemic infections (bacterial, fungal, viral, including infection caused by Herpes simplex, with eye damage); recent surgery or trauma to the nose; bleeding diathesis; frequent nosebleeds; children under 6 years of age (the safety and effectiveness of the drug have not been established). With caution: concomitant infectious disease requiring antibiotics; arterial hypertension; recent myocardial infarction; ulceration of the nasal septum; glaucoma; amoebiasis; severe liver failure; hypothyroidism; pregnancy; lactation period.

Side effects

Allergic reactions (urticaria, itching, angioedema, redness of the skin), nasopharyngeal infections caused by fungal flora, rhinorrhea, nosebleeds, burning sensation or irritation of the nasal mucosa, dry nose, sneezing, impaired sense of smell, increased eye pressure, glaucoma, disorder appetite, dyspepsia, nausea, diarrhea, headache, dizziness, arterial hypertension, difficulty breathing. When using nasal corticosteroids in large doses for a long time, cases of perforation of the nasal septum have been reported. Side effects characteristic of systemic corticosteroids may also appear during treatment with Aldecin if the drug is used in doses higher than recommended, or the patient is hypersensitive to corticosteroids, or has recently received therapy with systemic corticosteroids. If such symptoms and side effects develop, Aldecin should be discontinued, gradually reducing the dose.

Directions for use and doses

Intranasally, in the form of inhalations. Adults and adolescents over 12 years of age: the recommended dose is 1-2 inhalations into each half of the nose 4 times a day, depending on the severity of symptoms. Children from 6 to 11 years old: the recommended dose is 1-2 inhalations into each half of the nose 2 times a day, depending on the severity of symptoms. The maximum daily dose should not exceed 16 inhalations (0.8 mg) in adults and 8 inhalations (0.4 mg) in children aged 6 to 11 years. Once the therapeutic effect is achieved, the drug is discontinued, gradually reducing the dose. A guide for patients on proper inhalation Before using the nasal spray for the first time, it is necessary to “calibrate” it by pressing the dosing device 6-7 times. After “calibration”, a stereotypical supply of the medicinal substance is established. If the spray has not been used for 14 days or longer, then re-calibration is necessary before using again. Before inhalation, the nasal passages should be cleared. Shake the bottle vigorously. Insert the nasal applicator, holding it in a vertical position, into the nostril - towards the inner corner of the eye. In this case, the second nostril should be closed by pressing it with a finger, and the head should be slightly tilted forward. Do not spray the spray directly onto the nasal septum! Then you should take a deep breath through your nose while simultaneously pressing the spray and exhale through your mouth. Inhalation into the second nostril is performed in a similar way. To receive a single dose of 100 mcg, patients must inhale twice as described above.

Overdose

Symptoms: When used in very high doses, systemic effects of GCS may be observed, including suppression of adrenal function and symptoms of hypercortisolism. Treatment: if such symptoms appear, the dose should be reduced.

Interactions with other drugs

No clinically significant types of interaction of inhaled corticosteroids with other drugs have been identified. Phenobarbital, phenytoin, rifampicin and other inducers of microsomal oxidation reduce effectiveness. Methandrostenolone, estrogens, beta2-agonists, theophylline and orally administered corticosteroids enhance the effect. Increases the effect of beta-agonists.

Special instructions for use

Do not pierce the bottle nozzle with a needle or wire. It is necessary to protect the eyes from contact with the drug. The therapeutic effect of Aldecin, unlike local vasoconstrictors for the treatment of rhinitis, does not appear immediately when administered intranasally. Relief from rhinitis symptoms usually becomes noticeable within a few days of starting treatment. Patients who may have a decrease in immunity during GCS therapy should be warned about the danger of contact with certain infections (for example, chickenpox, measles) and the need to consult a doctor in case of such contact. This is of particular importance for children. Because the drug slows wound healing, patients who have recently had trauma or surgery to the nose should not use Aldecine Nasal Spray until the wounds have completely healed.

Upon completion of inhalation, it is recommended to rinse the mouth with running water.

Aldecin spray, instructions for use

From the age of six, you can use one or two injections in each nostril twice a day.

Nasonex for allergies

Nasonex is used to treat seasonal or year-round allergic rhinitis in adults and adolescents over 12 years of age. The recommended preventive and therapeutic dose of the drug is 2 inhalations (50 mcg each) into each nasal passage once (total daily dose of mometasone - 200 mcg/day). Clinical improvement after the first use of Nasonex is observed within 12 hours.

After achieving the desired therapeutic effect and to maintain it, the dose is reduced to 100 mcg/day. (one inhalation into each nostril once).

If the symptoms of the disease do not decrease, then, in agreement with the attending physician, it is possible to increase the daily dose to 400 mcg/day. When the patient's condition improves, the dose of the drug must be reduced.

For children aged 2-11 years with allergies, it is recommended to take one inhalation in each nasal passage once. The total dose is 100 mcg/day.

Interaction

Aldecine allows you to restore the patient's response to the use of beta-agonists , which significantly reduces the frequency of their use.

There are no significant drug interactions between inhaled glucocorticosteroids and other medications.

Inducers of microsomal oxidation ( rifampicin , phenobarbital, phenytoin) reduce the effectiveness of beclomethasone.

Estrogens, methandrostenolone , theophylline, beta2-adrenergic stimulants enhance the effect of the drug Aldecin.

Pharmacological action of Aldecin

Aldecin has antiallergic and anti-inflammatory effects, has an immunosuppressive effect (suppresses the immune system). The active substance beclomethasone inhibits the release of inflammatory mediators from mast cells, thereby reducing swelling of the mucous membrane and reducing the production of secretions (mucus). The drug also has a beneficial effect on mucociliary clearance (improves the functioning of the ciliated epithelium, which promotes the removal of mucus).

Aldecin spray has a bronchodilator effect (relaxes the smooth muscles of the bronchi) and reduces their hyperreactivity. Aldecine has virtually no systemic effect on the adrenal cortex, which is typical for oral administration of glucocorticosteroids. The drug restores the patient's response to drugs that dilate the bronchial muscles. For hormone-dependent bronchial asthma, Aldecin allows you to reduce the dose of glucocorticoids taken orally.

special instructions

Aldecine is not used to relieve acute attacks of bronchial asthma. Patients must be explained that the drug has a preventive effect, and that to achieve maximum effect, the drug must be used regularly, including in the absence of signs of asthma.

Regular use of the inhaler can improve respiratory function after just a week of treatment. The reason for the lack of effect may be the increased content of mucus and sputum in the respiratory system, as well as as a result of severe bronchospasm, which prevents beclomethasone from penetrating into the area of action.

Before prescribing Aldecin, the doctor must instruct the patient about the aspects of proper use of the inhaler, which will subsequently provide a better therapeutic effect due to the complete penetration of the main substance into the lungs.

When transferring a patient from an oral glucocorticosteroid to Aldecin, regular peak flow measurements .

Washing after using the drug helps prevent damage to the nose and eyelid skin.

Analogs of the drug Nasonex

Synonyms for Nasonex spray with the same active ingredient are: Desrinit, Rizonel, Asmanex Twistheiler, Momat Rino and Nosephrine.

Nasonex spray analogues

Analogues of Nasonex with a similar therapeutic effect (dosage form: spray): Flutinex, Avamis, Flixonase, Beclonazal, Beclomethasone, Fluticasone, Tafen, Rinoklenil, Polydexa, Nasobek, Nazarel, Budoster, Aldecin.

Nasal drops with GCS: Benacap, Benarin.

Before replacing Nasonex, you should definitely consult with your doctor regarding contraindications, dosage and duration of therapy.

Nasonex or Avamis?

Nasonex and Avamis have similar therapeutic effects, routes of administration and indications. The active component of Avamis spray is fluticasone furoate, Nasonex is mometasone furoate. Both substances are characterized by a very high degree of affinity for GCS receptors and exceptional local activity. However, among all existing corticosteroids for intranasal administration, mometasone has the lowest bioavailability and the fastest development of the therapeutic effect. In addition, Nasonex based on mometasone is approved for use in children from the age of two, while Avamis in pediatric practice is used only for the treatment of children over six years of age. Mometasone furoate has a more gentle effect on the body and does not have a negative effect.

But Avamis has fewer contraindications than Nasonex spray.

Do not forget that, despite the similarity of these two medications, only the attending physician can prescribe or replace one drug with another.

Nasonex or Desrinit?

The main differences between Nasonex and Dezrinit are price and manufacturer. Nasonex is an original drug produced in Belgium. And Dezrinit is a generic produced in Israel. The original drug always contains components of a higher degree of purification. Therefore, its effectiveness is higher and its tolerability is better. The active substance of the drugs is identical - mometasone. The composition contains minor differences in auxiliary components that do not affect the overall therapeutic effect.

Nasonex is more often used for children, because the drug has less side effects than its analogue.

Remember that even if the composition is the same, taking medications must be coordinated with your doctor, who will suggest the optimal solution for a specific disease.
