Otrivin

Otrivin (xylometazoline hydrochloride) is a vasoconstrictor-decongestant for use in otorhinolaryngological practice for rhinitis (including allergic origin), sinusitis, and otitis. Method of application: intranasal. The group of nasal decongestants is united by the property of constricting the blood vessels of the nasal mucosa. As a result, nasal breathing is facilitated and excessive mucus secretion is eliminated. A feature of vasoconstrictors is that their use is limited to five to seven days (primarily this applies to nasal drops, which are more difficult to dose than sprays). One of the most popular drugs in this group among domestic consumers is Otrivin, which, in addition to xylometazoline, contains moisturizing ingredients - sorbitol, which normalizes the degree of hydration of the mucous membrane and prevents it from drying out, as well as methylhydroxypropylcellulose, which potentiates the effects of sorbitol and has its own moisturizing effect due to changes in rheological properties solution. Otrivin is an effective drug that facilitates nasal breathing without irritating the mucous membrane and without disrupting the functioning of the ciliated epithelium lining it. Unlike its “colleagues” in the pharmacological group, it has a long-lasting (up to 10-12 hours) effect and is indicated for use within 14 days (as opposed to other decongestants, which cannot be used for more than one week). These features of the drug distinguish it from the general series and allow it to be considered a leader among other vasoconstrictor-decongestants.

It is well tolerated, incl. in persons with hypersensitive mucous membranes, and its acid-base balance corresponds to the physiological values ​​characteristic of the nasal cavity. Menthol and cineol, which are part of the drug, have a cooling effect and bring a feeling of freshness to the mucous membrane. When used in doses recommended by the manufacturer, it does not irritate the mucous membrane and does not increase blood flow to the blood vessels located near it. The therapeutic effect of the drug is noted already in the second minute of its instillation into the nasal cavity. When used intranasally in therapeutic doses, it is not absorbed into the systemic circulation. Contraindicated in persons with arterial hypertension, individual intolerance to xylometazoline and auxiliary components of the drug, advanced atherosclerosis, hyperthyroidism, glaucoma, as well as pregnant and lactating women. The drug can be used in pediatric practice under the supervision of a doctor (for children under one year old), adults (for children under 6 years old). Otrivin should be used with caution in elderly patients. If the recommended duration of use of the drug is exceeded, the development of so-called “rebound effect” (increased signs of rhinitis).

Excipients

Indications

  • acute respiratory diseases with symptoms of rhinitis;

Contraindications for use

  • hypersensitivity to xylometazoline and other components of the drug;
  • arterial hypertension;
  • tachycardia;
  • severe atherosclerosis;
  • glaucoma;
  • dry rhinitis or atrophic rhinitis;
  • hyperthyroidism;
  • condition after transsphenoidal hypophysectomy;
  • surgical interventions on the meninges (history);
  • children's age up to 12 years.

Carefully:

diabetes; pheochromocytoma; cardiovascular diseases (including ischemic heart disease, angina pectoris); prostatic hyperplasia; with increased sensitivity to adrenergic drugs, accompanied by insomnia, dizziness, arrhythmia, tremor, increased blood pressure.

pharmachologic effect

Xylometazoline has an alpha-adrenomimetic effect: it causes a constriction of the blood vessels of the nasal mucosa, eliminating swelling and hyperemia of the nasopharyngeal mucosa. Xylometazoline also reduces the accompanying hypersecretion of mucus and facilitates the drainage of nasal passages blocked by secretions and thus improves nasal breathing during nasal congestion.

Otrivin® is well tolerated by patients with sensitive mucous membranes; its effect does not interfere with mucus secretion. Otrivin® has a balanced pH value characteristic of the nasal cavity.

The menthol and eucalyptol contained in the preparation have a cooling effect on the nasal mucosa, giving a feeling of freshness.

In therapeutic concentrations, the drug does not irritate the mucous membrane and does not cause hyperemia.

The effect occurs 2 minutes after application and continues for 12 hours.

In vitro studies have shown that in humans, xylometazoline inhibits the infectious activity of rhinovirus, which causes the common cold.

Drug interactions

Xylometazoline is contraindicated in patients currently receiving MAO inhibitors or who have received them within the previous 2 weeks.

The simultaneous use of tri- or tetracyclic antidepressants and sympathomimetic drugs may lead to an increase in the sympathomimetic effect of xylometazoline, so this combination is recommended to be avoided.

Dosage regimen

Intranasally.

Adults and children over 12 years of age:

1 injection into each nasal passage 3 times a day.
Should not be used more than 3 times/day.
It is recommended to apply the last time just before bedtime.

Overdose

Symptoms:

Xylometazoline, when applied topically in an excessive dose or accidentally ingested, can cause severe dizziness, increased sweating, a sharp decrease in body temperature, headache, bradycardia, increased blood pressure, respiratory depression, coma and convulsions. Following an increase in blood pressure, a sharp decrease may be observed.

Treatment:

Appropriate supportive measures should be taken if overdose is suspected, and in some cases immediate symptomatic treatment under medical supervision is indicated. These measures should include monitoring the patient for several hours. In case of severe poisoning with cardiac arrest, resuscitation measures should last at least 1 hour.

The presenters recommended drugs containing the substance ipratropium bromide (not a specific drug). Since my situation was already critical (my nose had simply gone crazy), and it was possible to see an otolaryngologist only in a week, I relied on advice in this program and began to look for a drug containing the substance mentioned above. It turned out that there are only two such drugs (in the release form I needed). I made a choice in favor of Otrivin Complex. This spray contains ipratropium bromide and a vasoconstrictor component. Before I start writing about my experience with the medicine, I want to write about contraindications. Do not use the drug under any circumstances if at least one point applies to you. Contraindications: 1. Hypersensitivity to atropine and similar compounds. Allergic and anaphylactic reactions to this pharmaceutical group. 2. Glaucoma. 3. Surgical interventions in the meninges. 4. Pregnancy. 5. Breastfeeding period. 6. Age up to 18 years. Please don't give it to your children. The medicine is very powerful. God saves man, who save himself. You can find other contraindications and side effects in the instructions. If you read instructions online, read several options, as there are instructions that are incomplete. The instructions included with the Otrivin Complex itself are very detailed. Thanks to the manufacturer.

My impressions. I used the drug for several courses and when symptoms of vasomotor rhinitis occurred. Since 2016 I have bought several bottles.

First purchase: My condition: clogged frontal and sinuses, severe inflammation, headache, pain in the eyes and sinuses, runny nose, sneezing, insomnia, other medications (including corticosteroids) stopped helping. Pros: 1. Five minutes after the first use, the swelling subsided. 2. I managed to blow most of the mucus out of my nose. 3. Air exchange between the nose and nasopharynx was normalized. 4. The nose has stopped running. 5. The flow from the nose into the nasopharynx and along the back wall has stopped. 6. Itching and sneezing stopped. 7. More oxygen began to flow into the brain. 8. The headache disappeared (almost immediately). 9. The sinus pain went away on the second day. 10. Sleep has returned to normal. 11. The sense of smell has returned. And how! 12. The effect of the drug lasts for 5.5 - 6 hours. As the manufacturer claims. 13. After 3-4 days I felt like an absolutely healthy person. But the quality of life improved after the first use. 14. Shelf life 3 years. 15. Powerful injection device. The medicine even gets into the nooks and crannies of the nose. Power and pressure help clear even the frontal sinuses. 16. The spray does not flow out of the nose or flow down the back wall. 17. The spray is very fine. 18. If you follow the methods of use, the medicine is enough for 2-3 courses. A course of seven days is half a bottle. 19. Since the bottle is sealed, it can be used for a long time after the first use. 20. The dispenser is equipped with a spring. This determines its power. + It is long enough and has the right thickness. Anatomical, I would say.

This is what he looks like. 21. None of the manufacturers (neither in advertising nor in instructions) talk about one very unpleasant symptom. There are conditions when the nose does not flow, one can breathe quite well, but a mucous substrate flows down the back wall. This is only half the story. There is a constant reflex expectoration, which becomes a habit that irritates others, and even the patient himself. From constant expectoration, the throat becomes inflamed, it is sore, it hurts, the mucous membrane becomes loose and retains even more mucus and clots of snot. It turns out to be some kind of vicious circle. Otrivin Complex solved this problem after the first use. 22. A week later I saw a doctor. In my case, he said that the medicine was beneficial. The photo was very good.

Unfortunately, there was something I didn't like. Disadvantages and side effects: 1. Tachycardia. The pulse reached 95 beats per minute. My norm is 75.

2. Dry nose. 3. The appearance of crusts of snot interspersed with blood. 4. Dry mouth. As a result, a sore throat. I solved this problem by drinking plenty of water. 5. Not strong, dull pain behind the sternum. Occasionally. 6. Slight dizziness and blurred vision after injection of the drug. It lasts about 20-30 minutes, but I wouldn’t recommend getting behind the wheel. 7. With an increased pulse, a slight tremor in the hands.

The side effects went away on their own after stopping the medication. I used the rest of the bottle when symptoms of vasomotor rhinitis appeared or preventively when there was a likelihood of their occurrence. I held out for a long time. More than six months. But here it’s autumn - colds - exacerbation - what I wrote about at the beginning of my long review. I went to the nearest pharmacy. My drug was not available, but there was the second one, which I did not choose last time. At a discount, with a higher concentration of active ingredients. And it seemed to work the first time, but then miracles began. The drug worked in one go. It turned out that the problem was a very minor part - the spray dispenser. It lacked power, fineness of atomization and a little bit of length.

Realizing this, I bought Otrivin Complex again. I won’t dwell on the advantages. They are described above in great detail. I'll tell you briefly about the side effects this time. If I didn’t mention something, it means that I didn’t encounter such side effects this time.

Second purchase / second serious use. My condition: severe congestion, insomnia, runny nose. Cons: 1. There was tachycardia, but less often. 2. A vessel in my nose burst a couple of times. Covered lightly. There was no such nosebleed. 3. Less dry nose. My throat hurt. 4. I used the drug longer. Seven days. The maximum period of application mentioned by the manufacturer. A day later, congestion appeared. I sprayed the medicine into my nose twice a day. I didn't get such a clear effect. Then the mucous membrane swelled even more. Do not use the drug for longer to avoid addiction, deterioration of the mucous membrane and reverse effects. 5. This time the sense of smell was restored, but it was not as acute as in the previous use.

Despite the fact that the drug has many side effects, I will give it a solid A. It copes with the assigned tasks, completely eliminates the stated symptoms, and improves the quality of life. He helped me. I recommend it. I recommend it as an aid in the complex treatment of nasal diseases. I recommend it to people suffering from vasomotor rhinitis. But to prevent Orivin Complex from turning into a “poison,” please consult your doctor and carefully read the contraindications.
