Non-steroidal anti-inflammatory drugs have become an integral part of life. They help relieve pain, fight inflammation, and normalize body temperature. Amelotex belongs to the NSAID group, and, like all drugs in this category, has its contraindications.

Main substance

The main active ingredient is meloxicam. It is a representative of the group of non-selective cyclooxygenase inhibitors, that is, it blocks the production of prostaglandins, the number of which increases significantly during the progression of inflammatory processes.

The solution in ampoules may have a greenish tint, although it is mostly transparent. Amelotex does not contain hormones and belongs to the group of non-steroidal (non-hormonal) anti-inflammatory drugs.

Indications for use

For all drugs in the NSAID group, the direction of action comes down to three basic principles:

  • reduce fever;
  • eliminate pain;
  • relieve inflammation.

But each individual drug in this group has a more pronounced effect and is less active in relation to others. Amelotex works well in reducing the severity of pain in arthritis and arthrosis. In addition, this remedy successfully fights inflammatory processes, so the main indications for use are:

Amelotex does not treat the disease that caused pain and inflammation in the joint. It is a means of symptomatic therapy, allowing you to relieve unpleasant pain while the medication is in effect.

Despite the fact that Amelotex is a drug of the NSAID group, it is not prescribed for fever. It cannot be called an effective antipyretic. Meloxicam does not have a pronounced effect on the thermoregulation center, and therefore its antipyretic abilities are extremely ineffective. The main focus of the product is pain in diseases of the joint tissue.

I will only indicate what medicinal dose of Amelotex tablets my mother uses and whether they help her or not, that is, whether there is an analgesic effect. Most often, she takes these tablets in the morning, drinks one 7.5 mg tablet and the pain-relieving effect lasts exactly for a day, but this is when the pain is not severe. Be sure to take medicine if there is no pain, but it is going somewhere (pills help a lot). For severe pain, the single dose is increased to two tablets, that is, 15 mg per day.

I can say that my mother was satisfied with the analgesic effect of the drug “Amelotex”, she has been taking it for exactly a year, she is not going to look for a replacement yet, since the analgesic effect is good, and the drug costs mere pennies compared to the same “Meloxicam” (the difference in price is a decent three times).

I once wrote that I have a younger brother, and he takes these pills during attacks of renal colic (sand in the kidneys) and attacks of radiculitis, and says that the drug relieves acute pain well in both cases.

Contraindications

Due to the fact that Amelotex is a non-selective COX-1 inhibitor, its use, along with symptomatic treatment, carries a number of side effects, on the basis of which contraindications are formed:

  • patient age over 60 years;
  • ulcerative colitis;
  • active or history of gastric bleeding;
  • erosion of the gastric mucosa;
  • kidney failure requiring dialysis;
  • gestation period;
  • lactation period;
  • liver diseases in the active phase;
  • patient age under 15 years
  • bleeding from the rectum.

Long courses of taking the drug are unacceptable. In case of prolonged inflammation or prolonged pain, it is necessary to consult a specialist to identify the cause and prescribe adequate treatment for joint diseases.

Special conditions of admission

If symptomatic treatment cannot be carried out without the participation of non-steroidal anti-inflammatory drugs of the first group, it is necessary to assess the risks of side effects, especially in patients over 65 years of age. Patients with a high risk of adverse reactions from the gastrointestinal tract and older patients should be prescribed a drug that protects the mucous membranes of the gastrointestinal tract from damage as a complex therapy. Pharmacodynamics

AMELOTEX® gel contains meloxicam, a non-steroidal anti-inflammatory drug (NSAID), which has an analgesic and anti-inflammatory effect. The anti-inflammatory effect is associated with inhibition of the enzymatic activity of cyclooxygenase-2 (COX-2), which is involved in the biosynthesis of prostaglandins in the area of ​​inflammation. To a lesser extent, meloxicam acts on cyclooxygenase-1 (COX-1), which is involved in the synthesis of prostaglandin, which protects the mucous membrane of the gastrointestinal tract and takes part in the regulation of blood flow in the kidneys.

Meloxicam is a “chondroneutral” drug, does not have a negative effect on cartilage tissue, and does not affect the synthesis of proteoglycan by chondrocytes of articular cartilage.

When applied topically, the drug reduces or eliminates pain in the area where the gel is applied, including joint pain at rest and during movement. Helps increase range of motion.

Pharmacokinetics

Studies on rabbits have shown that meloxicam, when applied cutaneously in gel form, is characterized by prolonged transdermal absorption, prolonged circulation in the blood and gradual elimination, significantly different in kinetic characteristics from the intramuscular route of administration of the drug. There is no data indicating significant absorption of meloxicam into the systemic circulation.

In the systemic circulation, meloxicam binds tightly to plasma proteins, mainly albumin (99%). It undergoes biotransformation in the liver with the formation of inactive metabolites. It is excreted mainly in the form of metabolites in urine and feces in approximately equal proportions.

Indications

Symptomatic treatment of osteoarthritis accompanied by pain.
Post-traumatic pain syndrome (traumatic bruises, dislocations, damage to ligaments, muscles and tendons, muscle ruptures without compromising the integrity of the skin).

Used for symptomatic therapy, reducing pain and inflammation; does not affect the progression of the disease.

Contraindications

Amelotex injections instructions

Intramuscular injection is required to a depth exceeding 2/3 of the needle length. The drug is administered slowly, evenly applying pressure to the syringe plunger. There should be no sudden pushes of the medication stream into the mouse. After administration of the entire dose of the drug, a compaction of small diameter may remain. If the lump does not go away within 24 hours, it is necessary to apply alcohol compresses at the site of the lump. Otherwise, there is a risk of developing a post-injection abscess and, as a result, suppuration of the muscle at the injection site.

For patients at risk, I prescribe no more than 7.5 mg of the drug per day. For patients without a history of gastrointestinal diseases, aged 20 to 50 years, the daily dose can be increased to 15 mg.

Amelotex®

Patients with a history of gastrointestinal diseases should be regularly monitored by their physician. If ulcerative lesions of the gastrointestinal tract or gastrointestinal bleeding occur, AMELOTEX® should be discontinued.

Gastrointestinal ulcers, perforation, or bleeding may occur at any time during the use of NSAIDs, with or without warning symptoms or a history of serious gastrointestinal complications.

The consequences of these complications are generally more serious in older people.

To reduce the risk of developing adverse events from the gastrointestinal tract, the minimum effective dose should be used for the shortest possible short course.

When using the drug AMELOTEX®, serious skin reactions such as exfoliative dermatitis, Stevens-Johnson syndrome, toxic epidermal necrolysis syndrome may develop. Therefore, special attention should be paid to patients reporting the development of adverse events from the skin and mucous membranes, as well as hypersensitivity reactions to the drug, especially if such reactions were observed during previous courses of treatment. The development of such reactions is observed, as a rule, during the first month of treatment. If the first signs of a skin rash, changes in the mucous membranes or other signs of hypersensitivity appear, discontinuation of the use of AMELOTEX® should be considered.

Cases have been described when taking NSAIDs to increase the risk of developing serious cardiovascular thrombosis, myocardial infarction, angina, possibly fatal. This risk increases with long-term use of the drug, as well as in patients with a history of the above diseases and predisposed to such diseases.

NSAIDs inhibit the synthesis of prostaglandins in the kidneys, which are involved in maintaining renal perfusion. The use of NSAIDs in patients with reduced renal blood flow or reduced circulating blood volume may lead to decompensation of latent renal failure. After discontinuation of NSAIDs, renal function usually returns to baseline levels.

Those most at risk for developing this reaction are elderly patients, patients with dehydration, chronic heart failure, cirrhosis of the liver, nephrotic syndrome or acute renal impairment, patients concomitantly taking diuretics, ACE inhibitors, and patients undergoing major surgery. interventions that lead to hypovolemia. In such patients, diuresis and renal function should be carefully monitored when initiating therapy.

The use of NSAIDs in combination with diuretics can lead to sodium, potassium and water retention, as well as a decrease in the natriuretic effect of diuretics. As a result, predisposed patients may experience increased symptoms of heart failure or hypertension. Therefore, such patients should be closely monitored and adequate hydration maintained. Before starting treatment, a kidney function test is necessary. In case of combination therapy, renal function should also be monitored.

When using the drug AMELOTEX® (as well as most other NSAIDs), episodic increases in the activity of transaminases in the blood serum or other indicators of liver function are possible. In most cases, this increase was small and transitory. If the identified changes are significant or do not decrease over time, the drug AMELOTEX® should be discontinued and the identified laboratory changes should be monitored.

Weakened or malnourished patients may be less able to tolerate adverse events and should therefore be monitored closely.

Like other NSAIDs, AMELOTEX® may mask the symptoms of an underlying infectious disease.

As a drug that inhibits cyclooxygenase/prostaglandin synthesis, AMELOTEX® may affect fertility and is therefore not recommended for women who have difficulty conceiving. In this regard, in women undergoing examination for such problems, it is recommended to discontinue taking AMELOTEX®.

The drug contains 2.45 g of sorbitol in the maximum recommended daily dose. Patients with hereditary fructose intolerance are not recommended to take the drug.

In case of simultaneous use of anticoagulants for oral administration, ticlopidine, heparin for systemic use, thrombolytic agents, careful monitoring of the effect of anticoagulants is necessary.

It is recommended to monitor lithium levels during the period of administration of the drug AMELOTEX®, when changing the dose of lithium drugs and when discontinuing them.

In case of simultaneous use with methotrexate, strict control of the number of blood cells is recommended.
