Severe cardiovascular complications have been reported while taking vardenafil, including sudden death, tachycardia, myocardial infarction, ventricular tachyarrhythmia, angina pectoris and cerebrovascular accidents (including transient ischemic attack and cerebral hemorrhage). The majority of patients in whom these complications have been reported had pre-existing cardiovascular risk factors. However, it is not possible to definitively determine whether these events are directly related to these risk factors, vardenafil use, sexual activity, or a combination of these or other factors.
Drugs for the treatment of erectile dysfunction should be used with caution in patients with anatomical deformities of the penis (curvature, cavernous fibrosis, Peyronie's disease) and in patients with diseases associated with an increased risk of developing priapism (such as sickle cell anemia, multiple myeloma, leukemia ).
The safety and effectiveness of concomitant use of vardenafil in the form of film-coated tablets and orally dispersible tablets or with other treatments for erectile dysfunction have not been studied. In this regard, the use of such combinations is not recommended.
The maximum dose of vardenafil (20 mg) may be less well tolerated in elderly patients (aged >65 years) (see sections "Dosage and Administration" and "Side Effects").
Effect on QT interval duration
With a single use of Levitra® in doses of 10 mg and 80 mg, there was an increase in the duration of the QT interval by an average of 8 msec and 10 msec, respectively. A single 10 mg dose of vardenafil when coadministered with 400 mg of gatifloxacin, an active substance with comparable effects on the QT interval, produced an additive QT effect of 4 msec compared with gatifloxacin alone. The clinical significance of these QT changes is unknown.
It is unknown how important this trend is for clinical practice and whether it can be generalized to all patients in all circumstances, since much will depend on individual risk factors and susceptibility in each particular case. In this regard, the use of Levitra should be avoided in patients with relevant risk factors, such as hypokalemia, congenital prolongation of the QT interval, and in patients taking class IA (eg, quinidine, procainamide) or class III antiarrhythmic drugs (eg, amiodarone, sotalol).
The safety and effectiveness of Levitra® in combination with other methods of treating erectile dysfunction have not been studied, so their combined use is not recommended.
Effect on vision
Cases of visual impairment and nonarteritic anterior ischemic optic neuropathy (NAIOPN) have been reported while taking Levitra® and other PDE5 inhibitors. Analysis of observational data indicates an increased risk of acute NSAIDs in men with erectile dysfunction after use of PDE5 inhibitors such as vardenafil, tadalafil and sildenafil. Because this may be true for all patients taking vardenafil, patients should be advised that if sudden vision loss occurs, they should stop taking Levitra® and consult their doctor immediately.
Combined use with alpha-blockers
Combination therapy with alpha-blockers and Levitra® may be accompanied by the development of arterial hypotension with a corresponding clinical picture, since these drugs have a vasodilating effect. The combined use of vardenafil and alpha-blockers is permissible only in the presence of stable blood pressure while taking alpha-blockers, and Levitra® should be taken in the minimum recommended dose of 5 mg. You should not take Levitra® at the same time as alpha-blockers, with the exception of tamsulosin, which may be taken at the same time as taking Levitra®. A time interval should be observed between taking vardenafil and other alpha-blockers. Alpha-blocker therapy should be started with a minimum dose. A gradual increase in the dose of alpha-blockers in patients receiving drugs from the PDE-5 inhibitor group may lead to a further decrease in blood pressure.
Concomitant use with CYP3A4 inhibitors
The simultaneous use of vardenafil with strong CYP3A4 inhibitors, such as itraconazole and ketoconazole (in oral dosage forms), should be avoided, since very high plasma concentrations of vardenafil are achieved when combined with these drugs.
Coadministration with grapefruits or grapefruit juice is expected to increase vardenafil plasma concentrations. Taking this into account, such combinations should be avoided (see section “Interaction with other drugs”). Concomitant use with HIV protease inhibitors, such as indinavir and ritonavir, and medications containing cobicistat and their combinations is contraindicated.
Effect on bleeding risk
Since Levitra® has not been used in patients with a tendency to bleed or in patients with exacerbation of peptic ulcer disease, its use in these cases is possible only after a careful assessment of the benefit-risk ratio.
The drug Levitra® does not affect the duration of bleeding, nor does it affect this indicator when used in combination with acetylsalicylic acid.
Vardenafil does not enhance platelet aggregation caused by various drugs. At a concentration above the therapeutic level, vardenafil causes a slight increase in the antiplatelet effect of sodium nitroprusside, which is a donor of nitric oxide. In studies in rats, the combined use of vardenafil and heparin showed no effect on bleeding time, but the use of this combination in humans has not been studied.
