Gentamicin is an antibiotic that exhibits a wide spectrum of effects and belongs to the group of aminoglycosides. In the body, it binds to the 30S subunit of ribosomes, as a result of which protein synthesis is disrupted and the production of the transport and messenger RNA complex is suspended. RNA is misread and nonfunctional proteins are produced. A bactericidal effect is observed - at high concentrations of the substance, it reduces the barrier functions of cytoplasmic membranes, as a result of which microorganisms die.
There is a high sensitivity to this antibiotic on the part of some gram-negative microorganisms.
Sensitivity to the substance of a number of gram-positive microorganisms was also noted.
Antibiotic resistance is demonstrated by: Neisseria meningitidis, Providencia rettgeri, Clostridium spp., Treponema pallidum, Bacteroides spp., Streptococcus spp.
If Gentamicin is combined with penicillins , its activity is noted against Enterococcus faecium, Enterococcus faecalis, Enterococcus avium, Enterococcus durans, Streptococcus faecium, Streptococcus durans, Streptococcus faecalis.
Resistance of microorganisms to this drug develops slowly, but those strains that demonstrate resistance to neomycin and kanamycin may also be resistant to the effects of gentamicin. It has no effect on fungi, protozoa, or viruses.
Pharmacokinetics and pharmacodynamics
After intramuscular administration, rapid and complete absorption of the substance occurs. The maximum concentration in the body after intramuscular administration is achieved after 0.5-1.5 hours. After a 30-minute intravenous infusion - after 30 minutes, after a 60-minute intravenous infusion - after 15 minutes.
Little binds to plasma proteins - up to 10%. Therapeutic concentrations of the substance are found in the kidneys, liver, lungs, as well as in body fluids - peritoneal, ascitic, synovial, pericardial, pleural, lymphatic; found in pus discharged from wounds, granulations, and urine.
Low concentrations of the substance are observed in muscles, adipose tissue, breast milk, bile, bones, sputum, bronchial secretions, cerebrospinal fluid, and eye moisture.
It almost does not penetrate the BBB in adult patients; it penetrates the placenta.
Concentrations in the cerebrospinal fluid of newborns are higher than in adults.
It is not metabolized in the body. The half-life in adults is 2-4 hours, in children under 6 months - 3-3.5 hours.
It is mainly excreted from the body through the kidneys, unchanged; small amounts of the antibiotic are excreted in the bile. If the patient’s kidney function is normal, then during the first day 70-95% of the substance is excreted. In this case, a concentration of more than 100 mcg/ml is observed in the urine. Cumulation is observed upon repeated administration.
Pharmacological properties of the drug Gentamicin sulfate
Pharmacodynamics. Gentamicin is an aminoglycoside antibiotic with a broad spectrum of action. The mechanism of action is associated with inhibition of 30S ribosomal subunits. In vitro tests confirm its activity against different types of gram-positive and gram-negative microorganisms: Escherichia coli, Proteus spp. (indole-positive and indole-negative), Pseudomonas aeruginosa, Klebsiella spp., Enterobacter spp., Citrobacter spp., Salmonella spp., Shigella spp. and Staphylococcus spp . (including penicillin- and methicillin-resistant strains). The following microorganisms are generally resistant to gentamicin: Streptococcus pneumoniae , most other streptococcal species, enterococci, Neisseria meningitides, Treponema pallidum and anaerobic microorganisms such as Baacteroides spp . or Clostridium spp . Pharmacokinetics . Gentamicin is easily absorbed, reaching maximum plasma concentrations 30–60 minutes after intramuscular administration. Therapeutic concentrations in the blood remain for 6–8 hours. With intravenous drip administration, the concentration of the antibiotic in the blood plasma during the first hours exceeds the concentration achieved after intramuscular administration of the drug. Plasma protein binding is 0–10%. In therapeutic concentrations it is determined in kidney tissue, lungs, pleural and peritoneal exudates. Normally, gentamicin, when administered parenterally, penetrates the BBB poorly, but with meningitis, the concentration in the CSF increases. The drug passes into breast milk. About 70% of gentamicin is excreted unchanged into the urine during the day by glomerular filtration. The half-life from blood plasma is approximately 2 hours. If the excretory function of the kidneys is impaired, the concentration and half-life of gentamicin increases significantly.
Indications for use
Indications for use of the drug are diseases of an infectious-inflammatory nature that were provoked by microorganisms sensitive to gentamicin.
Parenteral use of the drug (4% solution) is indicated for the following diseases:

- cystitis;

Gentamicin injections, instructions for use
Before starting treatment, it is recommended to determine the sensitivity of microflora to gentamicin.

Pharmacological properties
The drug GENTAMICIN SULPHATE contains the active substance – gentamicin sulfate, which is used to treat infections caused by pathogens sensitive to it. GENTAMICIN SULFATE is intended for the treatment of the following infectious and inflammatory eye diseases: • acute and chronic conjunctivitis (inflammation of the mucous membrane of the eye); • corneal ulcer; • keratitis (inflammation of the cornea); • keratoconjunctivitis (inflammation of the cornea and conjunctiva); • acute and chronic iritis (inflammation of the iris); • acute and chronic blepharitis (inflammation of the edges of the eyelids); • blepharoconjunctivitis (inflammation of the edges of the eyelids and conjunctiva); • dacryocystitis (inflammation of the lacrimal sac of the eye). GENTAMICIN SULFATE is also used for the prevention and treatment of purulent complications after eye surgery.
Compound
One bottle contains the active ingredient: gentamicin sulfate – 15 mg; excipients: disodium edetate, solution of 60 mg/ml dextran with M.m. 50000-70000 in a solution of 9 g/l sodium chloride, water for injection.
Contraindications
Do not take the drug if: • you have hypersensitivity to the active substance or to any component of the drug (see section Composition); • you have hypersensitivity to other antibiotics from the aminoglycoside group; • You are pregnant or breastfeeding. Special instructions and precautions

Pregnancy, breastfeeding and fertility

Use of the drug

Recommendations for use

Overdose

Overdose
In case of an overdose of Gentamicin in ampoules or other forms of the drug, a decrease in neuromuscular conduction may be observed, up to respiratory arrest.
In case of overdose, adult patients must be administered anticholinesterase drugs ( Proserin ) and calcium supplements . Very rarely, the following side effects occur: nausea, increased salivation, loss of appetite, weight loss, purpura, laryngeal edema, joint pain, arterial hypotension and drowsiness, possible blockade of neuromuscular conduction and respiratory depression. Pain may occur at the site of intramuscular injection of gentamicin; with intravenous administration, phlebitis and periphlebitis may develop.
Interaction
If Gentamicin injections or the use of other forms of medication are practiced simultaneously with Vancomycin , aminoglycosides , cephalosporins , ethacrynic acid , the oto- and nephrotoxic effects may increase.
If the drug is used with Indomethacin , then the clearance of Gentamicin decreases, its concentration in the blood increases and, accordingly, the toxic effect increases.
When using Gentamicin with opioid analgesics , drugs for inhalation anesthesia, the likelihood of neuromuscular blockade increases, and apnea .
The concentration of gentamicin in the blood increases if it is taken simultaneously with loop diuretics .
Drug interactions Gentamicin sulfate
Concomitant use with potent diuretics (furosemide, ethacrynic acid) should be avoided, since the latter may enhance the ototoxic and nephrotoxic effect. Impaired respiratory function may occur due to neuromuscular blockade in patients who are simultaneously prescribed muscle relaxants (succinylcholine, tubocurarine, decamethonium), anesthetics, or a history of a previous massive blood transfusion using a citrate anticoagulant. The use of calcium salts and anticholinesterase drugs can eliminate the effects of neuromuscular blockade. Simultaneous and/or sequential systemic or local use of other neuro- and/or nephrotoxic drugs, such as cisplatin, cephaloridine, aminoglycoside antibiotics, polymyxin B, colistin, vancomycin should be avoided. The risk of renal dysfunction increases with simultaneous use of indomethacin and other NSAIDs, as well as quinidine, cyclophosphamide, ganglion blockers, verapamil, and polyglucin in combination with gentamicin. Gentamicin increases the toxicity of digoxin. With the simultaneous administration of aminoglycosides and penicillins, the half-life decreases and their content in the blood serum decreases. A decrease in the half-life occurs in patients with severe renal impairment when carbenicillin is combined with gentamicin. When mixing antibiotics of the aminoglycoside group with antibiotics of the beta-lactam group (penicillins, cephalosporins) in one volume, mutual inactivation is possible. Also, gentamicin is pharmacologically incompatible with amphotericin and heparin.
