Pharmacological properties
A laxative of the triarylmethane group, activated in the colon under the action of bacterial sulfatases; the substance that is released in this case stimulates the sensitive nerve endings of the intestinal mucosa, enhances its motility, and promotes the accumulation of water and electrolytes in the lumen of the colon. The result of this is stimulation of defecation, reduction of transit time and softening of stool. The use of the drug is not accompanied by tenesmus and intestinal spasms. After oral administration, it reaches the large intestine without significant absorption, so there is no intrahepatic circulation. Under the influence of bacterial flora, it breaks down to form the active form (free diphenol). The laxative effect develops 6-12 hours after administration; the severity of the laxative effect does not correlate with the level of active diphenol in the blood plasma. During a course of treatment, Picolax stimulates the growth and metabolic activity of normal intestinal microflora.
Indications for use
Constipation or cases requiring relief of bowel movements.
Directions for use and doses
Prescribed orally, preferably at night.
Adults and children over 10 years old – 5-10 mg 1 time per day. For severe and persistent constipation, adults are recommended to take 15 mg once.
Children from 4 to 10 years old – 2.5-5 mg 1 time per day.
For children, a single dose of the drug can be effective. If necessary, the course of treatment for the child can be extended, but it is preferable to limit it to three days. The duration of treatment is determined individually, usually it does not exceed 10 days.
Side effects
With short-term use of the drug, side effects are rare.
With long-term use of the drug in significantly increased doses, the following violations are possible:
from the water-electrolyte balance: loss of potassium, sodium and other electrolytes, possible development of dehydration;
from the digestive system: rarely - dyspeptic symptoms, vomiting, nausea, cramps and abdominal pain, diarrhea, flatulence, pain in the stomach and anus, increased intestinal motility, which disappear when the dose of the drug is reduced;

Contraindications
Hypersensitivity to the components of the drug; intestinal obstruction; acute inflammatory bowel diseases and acute diseases of the abdominal organs (including acute appendicitis, ulcerative colitis, peritonitis); abdominal pain, nausea and vomiting of unknown origin; condition after surgery on the gastrointestinal tract; severe dehydration.
In the case of rare hereditary conditions that may result in hypersensitivity to the excipients of the drug (see section “Special instructions”), taking the drug is contraindicated.
Appropriate safety precautions for use.
Like other laxatives, Picolax should not be used daily or for an extended period without identifying the cause of constipation. Long-term and excessive use can lead to fluid or electrolyte imbalance and hypokalemia. Dizziness and/or syncope have occurred in patients taking sodium picosulfate. Available evidence from these cases suggests that the events are consistent with defecatory syncope (or syncope associated with attempting to defecate) or a vasovagal response to abdominal pain associated with constipation and not necessarily sodium picosulfate ingestion.
Drug interactions
Electrolyte imbalance can lead to decreased tolerance to cardiac glycosides. Diuretics or glucocorticosteroids may increase the severity of hypokalemia caused by the use of Picolax in high doses. Simultaneous treatment with antibiotics may reduce the laxative effect of the drug.

Overdose
Symptoms High doses of Picolax may cause loose stools, abdominal cramps (colic) and clinically significant fluid and electrolyte imbalances (hypokalemia, loss of other electrolytes and fluids). There are reports of ischemia of the colon mucosa with the use of higher doses of Picolax, significantly higher than those usually recommended for constipation. Long-term overdose leads to the development of chronic diarrhea and abdominal pain, hypokalemia, secondary hyperaldosteronism, possible kidney damage and the development of nephrolithiasis, metabolic alkalosis and muscle weakness as a result of hypokalemia.
