Pharmacodynamics and pharmacokinetics
Rogaine solution is a medicine used to treat thinning scalp hair ( baldness , alopecia ). The main effect of minoxidil is aimed at stimulating and activating hair growth in patients suffering from androgenetic alopecia .
When using the drug externally, signs of hair growth were observed after at least 4 months of use. The onset of scalp growth and the degree of its severity are quite variable and depend on the personal response of each patient to the treatment. Rogaine 5% solution has a more significant effect on hair growth than a 2% solution of this drug, which has been proven by the observed increase in the number of vellus hairs .
When treatment with Rogaine is discontinued, the appearance of new hair slows down until the process stops completely and over the course of 3-4 months brings the scalp to the level observed before the onset of problems with hair loss.
The true mechanism of action of minoxidil in the treatment of androgenetic alopecia is still unknown.
When applied externally to intact scalp, absorption of minoxidil remains low. On average, less than 1.5% (with variability - 0.3-4.5%) of the drug from its total used dose enters the systemic circulation. Application of 1 ml of a 5% solution of Regaine (50 mg minoxidil ) leads to the absorption of only 0.70 mg of the drug. The effect of skin pathologies on the absorption process of minoxidil has not been studied.
The profile of metabolic transformations of minoxidil , when applied externally, has not yet been thoroughly studied. It is known that the drug does not bind to plasma proteins and does not penetrate the BBB.
When you stop using Rogaine, about 95% of minoxidil in the systemic circulation is excreted within 4 days, mainly through glomerular filtration in the urine.
Minoxidil
Minoxidil is a hair drug that is a vasodilator that effectively stops hair loss and increases the growth rate of new hair shafts. Minoxidil, for example, is the main component of the drug "Rogaine" or "Regaine", which is actively used to get rid of androgenetic alopecia and treat hair growth. Among the drugs that also include Minoxidil are eucapil, procopil, and alerana. It is used as an additive by many cosmetic lines that produce shampoos to strengthen hair.
Minoxidil directly affects hair follicles, activates cell nutrition by dilating blood vessels and improving blood flow. For ease of use, Rogaine is available in the form of a spray and hair foam.
Minoxidil is actively used to treat hair loss in women. Correctly, a drug with a 2% concentration is used. In turn, “Regaine” for men has a 5% percentage solution concentration. The product is especially effective in the initial stages of hair loss with aggressive processes, and gives less pronounced results with a large area of baldness.
Side effects
Local side effects
Negative side effects observed during clinical studies of the effects of Rogaine were, as a rule, limited to the formation of dermatitis of the scalp. of dermatitis , characterized by peeling , redness and inflammation , was much less common .
Itching of the scalp , allergic contact dermatitis alopecia , folliculitis , seborrhea and unwanted hair growth were occasionally observed
During a comparative clinical study, a comparison of 2% and 5% solutions of minoxidil and placebo showed a more frequent development of dermatological reactions in the group using the 5% drug. The severity and nature of negative side effects in the groups using 2% and 5% minoxidil were almost comparable, although the incidence of side effects was higher in the latter group.
Systemic side effects (less common than local)
- Skin: swelling of the face , nonspecific reactions of allergic etiology (including the formation of a skin rash and the occurrence of urticaria ).
- Respiratory system: allergic rhinitis , shortness of breath .
- Nervous system: vertigo; headaches , neuritis , dizziness .
- Cardiovascular system: fluctuations , chest pain, rapid heartbeat , swelling , changes in heart rate .
A direct cause-and-effect relationship between the use of Rogaine solution and the formation of the above adverse reactions was monitored only for dermatological manifestations .
Composition and analogues
Drugs similar in effect to Rogaine.
Rogaine contains minoxidil in varying concentrations. Its indicator depends on the form of release and purpose. Thus, 2% of the drug is usually recommended for women, and 5% for men or people with increased fat content of the epidermis on the head and advanced stages of baldness.
In addition to the main active ingredient, Rogaine hair product also consists of auxiliary elements:
- distilled water;
- ethanol;
- propylene glycol (only in lotion).
Modern analogues of this dermatotropic drug based on the active substance included in the composition are:
- Alopexy;
- Minoxidil (Minoxidil);
- Kosilon;
- Revasil;
- Generolon;
- Perfect Hair Solution;
- Alerana.
That is, many similar products are now produced by different companies. However, the hair loss drug Rogaine showed the world's first proven clinical effectiveness and to this day remains widely used in the treatment of baldness, as well as for beard growth. But you should know that not every form of alopecia can be treated.
ON A NOTE! This product was registered by the Ministry of Health of the Russian Federation on May 23, 1996. Registration number: P-8-242 No. 007196.

special instructions
Before starting treatment with Rogaine®, patients should undergo a general examination, including the collection and study of a medical history. The doctor must make sure that the scalp is healthy. Although the effects listed below have not been observed when Rogaine® is applied to the skin, minoxidil is absorbed through the skin in small quantities and there is a risk of systemic side effects such as water and salt retention, generalized and local edema, pericardial effusion, pericarditis, pericardial tamponade, tachycardia, angina pectoris, increased orthostatic hypotension caused by certain antihypertensive drugs, for example, guanethidine and its derivatives. Prescribing Rogaine® to patients with arterial hypertension receiving treatment with guanethidine or guanethidine derivatives, or minoxidil, is possible only under medical supervision. Patients should undergo periodic examinations to identify possible signs of systemic side effects of minoxidil. If systemic side effects occur, treatment should be discontinued. To treat edema and fluid retention in the body, diuretics can be prescribed if necessary. To eliminate tachycardia and angina pectoris, beta-blockers can be prescribed. Patients with a history of cardiovascular disease should be warned that treatment with Rogaine may cause an exacerbation of these diseases. If systemic side effects or severe skin reactions occur, patients should discontinue the drug and consult a doctor. The effect of Rogaine® in patients with impaired skin integrity and dermatoses of the scalp at the site of application of the drug is unknown. The safety and effectiveness of Rogaine® for patients under 18 years of age and over 65 years of age has not been established. Rogaine® contains ethyl alcohol, which can cause inflammation and irritation of the eyes. If the drug gets on sensitive surfaces (eyes, irritated skin, mucous membranes), it is necessary to rinse the affected area with plenty of cold water. Accidental ingestion of the solution can lead to serious consequences. The symptoms observed in this case mainly affect the cardiovascular system and include fluid retention, decreased blood pressure or tachycardia. Patients should be warned about the need to store Rogaine® (like other medicines) out of the reach of children. If the drug has become unusable or has expired, then it should not be poured into wastewater or thrown into the street; The drug should be placed in a bag and placed in a trash container. These measures will help protect the environment.
Side effects
Local side effects Side effects observed in clinical studies of Rogaine® were mostly dermatitis of the scalp. More severe cases of dermatitis, manifested by redness, peeling and inflammation, have been observed less frequently. In rare cases, scalp itching, allergic contact dermatitis, folliculitis, alopecia, unwanted hair growth and seborrhea have been reported. In a comparative clinical study examining the use of minoxidil solution 5%, 2% and placebo, dermatological reactions were observed most often in the group receiving minoxidil 5%. The nature and severity of side effects were comparable in the minoxidil 5% and 2% groups; The incidence of side effects was higher in the minoxidil 5% treatment group. Systemic Side Effects Less common side effects are grouped by organ system. Dermatological reactions: nonspecific allergic reactions (skin rash, urticaria), facial swelling. From the respiratory system: shortness of breath, allergic rhinitis. From the side of the central nervous system: headache, dizziness, vertigo, neuritis. From the cardiovascular system: chest pain, blood pressure fluctuations, rapid heartbeat, changes in heart rate, edema. A clear causal relationship between the use of Rogaine® and the occurrence of the side effects listed above has been established only in the case of dermatological reactions.
Drug interactions
There is a theoretical possibility of increased orthostatic hypotension in patients receiving concomitant treatment with peripheral vasodilators, which, however, has not received clinical confirmation. A very slight increase in minoxidil blood levels in patients with arterial hypertension and taking minoxidil orally cannot be ruled out when using Rogaine® simultaneously, although relevant clinical studies have not been conducted. It has been found that topical minoxidil may interact with some other topical medications. The simultaneous use of minoxidil solution for external use and a cream containing betamethasone (0.05%) leads to a decrease in the systemic absorption of minoxidil. Simultaneous use of a cream containing tretinoin (0.05%) leads to increased absorption of minoxidil. Simultaneous application of minoxidil to the skin and topical drugs such as tretinoin and dithranol, which cause changes in the protective functions of the skin, may lead to increased absorption of minoxidil.
Storage conditions
The drug should be stored out of the reach of children at a temperature not exceeding 25°C.
Overdose
, systemic side effects may occur due to the vasodilating effect of the active ingredient of the drug - minoxidil . It should be remembered that 5 ml of a 2% solution of Rogaine includes 100 mg of minoxidil , which corresponds to the maximum recommended daily dosage of the drug for the treatment of arterial hypertension , and 5 ml of a 5% solution includes 250 mg of minoxidil , which is 2.5 times higher than this dose. Symptoms of oral administration of large doses of minoxidil may include decreased blood pressure , fluid retention , and tachycardia .
In order to eliminate the observed fluid retention diuretics (diuretics) can be prescribed tachycardia can be treated with beta-blockers , and arterial hypotension with intravenous sodium chloride (0.9%). In this situation, the use of sympathomimetic drugs such as Epinephrine or Norepinephrine is inappropriate due to their excessive cardiac stimulating effects.
How does Rogaine (Rogaine) work?
The active ingredient in Rogaine is minoxidil. First of all, as a result of the effects of minoxidil, the growth phase (anagen) of hair is prolonged. In addition, minoxidil has a pronounced vasodilating effect, which significantly improves blood supply to the hair follicles. Minoxidil is a white or whitish crystalline substance. When applied topically, minoxidil has been noted to have a stimulating effect on hair growth in individuals with androgenetic alopecia (hair thinning, baldness).
Interaction
Theoretically, with the parallel use of Rogaine solution and peripheral vasodilators, it is possible to intensify existing orthostatic hypotension , but at this time there is no clinical confirmation of this interaction.
In the case of using Rogaine and parallel oral administration of minoxidil for the treatment of arterial hypertension , a slight increase in its serum content is theoretically possible, although relevant clinical studies in this direction have not been carried out.
Prescribing Rogaine solution to patients suffering from arterial hypertension and taking guanethidine or its derivatives is possible only under constant medical supervision.
It has been proven that the solution interacts with some other external (local) drugs. Thus, the combined use of Rogaine and 0.05% cream containing betamethasone led to a decrease in the absorption of minoxidil , and the parallel use of external agents with dithranol and tretinoin , which cause changes in the skin's protective functions, can cause an increase in the absorption of minoxidil .
Stop the painful process
Androgenic and androgenetic alopecia are treated! It is important to start using medications containing Rogaine on time. Penetrating through the pores of the skin, the active substance suppresses pathological processes in the follicles. With systematic use of Rogaine, the production of dihydrotestosterone is reduced. The bulbs are starting to come to life. Their transition from the sleep stage to the growth stage becomes noticeable after just a couple of months of use. Soft hairs appear first. They usually have a lighter tone than what the person previously had. At this stage, the main thing is not to stop! After 6 months of use, hairiness increases and the effect becomes pronounced. The optimal result can be achieved after 1 year of treatment. It is important to know exactly how to apply Rogaine. Compliance with instructions and systematic use are the main factors for successful treatment of alopecia.

special instructions
Before starting therapy with Regaine, potential patients should be given a general examination with the collection and study of a medical history . In case of a positive assessment of the patient’s general condition, it is necessary to examine the scalp and ensure its integrity.
Until now, with external use of Regaine, the following effects have not been observed, however, due to the penetration of a small amount of minoxidil into the systemic circulation, the following negative manifestations are potentially possible: salt and water retention, tachycardia , local and generalized edema , angina pectoris , pericarditis , pericardial effusion and tamponade, increased hypotension caused by taking certain antihypertensive drugs .
Throughout Regaine therapy, patients are required to undergo periodic examinations in order to identify possible negative systemic side effects of minoxidil . If such phenomena are detected, therapy is suspended until their causes are clarified.
Patients with a history of pathologies of the cardiovascular system, when using Rogaine, must be warned about the potential for exacerbation of their observed diseases.
If systemic side effects or severe skin reactions occur, patients should discontinue the drug and consult a doctor.
The safety and effectiveness of using Rogaine for the treatment of persons under 18 and after 65 years of age, as well as for the treatment of patients with dermatoses and disorders of the scalp, is unknown.
Ethyl alcohol included in the solution , if it gets on the mucous membranes of the eyes, can cause irritation and inflammation . In such a situation, you should immediately rinse your eyes with plenty of cold water.
When taking the solution orally, there is a high probability of serious negative manifestations, mainly from the cardiovascular system (including tachycardia , fluid retention , drop in blood pressure ). In this regard, storage of the drug requires a place inaccessible to children.
Composition of Rogaine for hair
Different manufacturers produce the drug with different concentrations. The degree of influence of Rogaine on the hair follicles is determined by the content of the active substance: from 2% to 5%. Ethanol and ordinary distilled water are used as auxiliary components in the preparations. The lotions also contain:
- propylene glycol;
- butylated hydroxytoluene;
- lactic and citric acid;
- glycerol;
- cetyl and steal alcohol;
- polysorbate 60.
The beneficial effects of Rogaine are manifested in the fact that the product opens pores and activates blood flow. Once the damaged bulb receives enough nutrients, it begins to generate new hair.

Reviews of Rogaine
Opinions about this drug, used to treat alopecia (baldness), are divided approximately equally and, in the vast majority of cases, the assessment of this drug comes from representatives of the fair half of humanity. Women's reviews of Rogaine are quite varied. For some, this solution simply became a “savior”, for others it helped stop hair loss , and for others it brought nothing but loss of time and money. Reviews from men about the drug Rogaine are generally no different from the opinions of women and indicate a rather selective effect of this drug.
Advantages of Finasteride in the treatment of hair loss
- Reviews about Finasteride are positive. The drug leads to a quick stop of hair loss. This has been proven by millions of men who have used the drug to treat alopecia for several years.
- The product is suitable for monotherapy and for use in combination with other drugs. Finasteride is most active in the initial stages of hair loss, and produces less pronounced results as alopecia progresses. A good effect is achieved if you use the product after hair transplantation.
- Finasteride, reviews of which indicate its maximum effectiveness, leads to a reduction in testosterone activity in the hair.
