Pharmacodynamics Essential phospholipids are the main elements of the structure of the cell membrane and cellular organelles of the liver. In liver diseases, there is always damage to the membranes of liver cells and their organelles, which leads to disruptions in the activity of associated enzymes and receptor systems, deterioration in the functional activity of liver cells and a decrease in the ability to regenerate. The phospholipids that make up the drug Esslial forte correspond in their chemical structure to endogenous phospholipids, but are superior to endogenous phospholipids in activity due to their higher content of polyunsaturated (essential) fatty acids. The integration of these high-energy molecules into damaged areas of hepatocyte cell membranes restores the integrity of liver cells and promotes their regeneration. The cis-double bonds of their polyunsaturated fatty acids prevent the parallel arrangement of hydrocarbon chains in the phospholipids of cell membranes; the density of the phospholipid structure of the cell membranes of hepatocytes decreases, which causes an increase in their fluidity and elasticity, and improves metabolism. The resulting functional blocks increase the activity of enzymes fixed on membranes and contribute to the normal physiological pathway of the most important metabolic processes. Phospholipids, which are part of the drug Esslial forte, regulate the metabolism of lipoproteins, transferring neutral fats and cholesterol to sites of oxidation, mainly by increasing the ability of high-density lipoproteins to bind to cholesterol. Thus, there is a normalizing effect on the metabolism of lipids and proteins, on the detoxification function of the liver and phospholipid-dependent enzyme systems, which ultimately prevents the formation of connective tissue in the liver and promotes the natural restoration of liver cells. With the excretion of phospholipids into the bile, the lithogenic index decreases and the bile is stabilized.
Pharmacokinetics More than 90% of phospholipids taken orally are absorbed in the small intestine. Most of them are cleaved by phospholipase A to 1-acyl-lysophosphatidylcholine, 50% of which is immediately reverse acetylated into polyunsaturated phosphatidylcholine during the absorption process in the intestinal mucosa. This polyunsaturated phosphatidylcholine enters the bloodstream through the lymph, and from there, mainly bound to high-density lipoproteins, enters the liver. Pharmacokinetic studies in humans were conducted using radiolabeled dilinoleyl-phosphatidylcholine (3H and 14C). A combined drug whose effect is determined by the properties of its constituent components. “Essential phospholipids” are complex substances of natural origin, which are diglyceride esters of phosphatidic and unsaturated fatty acids (mainly oleic and linolenic). They are an important structural element of cell membranes and membranes of hepatocyte organelles. Phospholipids regulate membrane permeability and the activity of membrane-bound enzymes, ensuring normal processes of oxidative phosphorylation. The drug normalizes the biosynthesis of phospholipids and general lipid metabolism in case of damage to hepatocytes of various etiologies. Restores hepatocyte membranes through structural regeneration and through competitive inhibition of oxidative processes: being embedded in biomembranes, unsaturated fatty acids are able to take on toxinogenic effects instead of membrane lipids. Promotes the regeneration of liver cells, stabilizes the physical and chemical properties of bile. Thiamine mononitrate (vitamin B1) as a coenzyme is involved in carbohydrate metabolism. Riboflavin (vitamin B2) is the most important catalyst for cellular respiration processes. Pyridoxine hydrochloride (vitamin B6) as a coenzyme takes part in the metabolism of amino acids and proteins. Cyanocobalamin (vitamin B12), together with folic acid, is involved in the synthesis of nucleotides. Nicotinamide is involved in the processes of tissue respiration, fat and carbohydrate metabolism. Pharmacokinetics
When taken orally, more than 90% of phospholipids are absorbed from the gastrointestinal tract. The main amount is broken down to 1-acyl-LYSO-phosphatidylcholine, 50% of which is converted into polyunsaturated phosphatidylcholine in the small intestine. Next, polyunsaturated phosphatidylcholine in combination with HDL is transported to the liver. The maximum concentration of phospholipids in the blood plasma is achieved after 6 hours. Tocopherol acetate enhances the effect of steroidal and non-steroidal anti-inflammatory drugs (diclofenac sodium, ibuprofen, prednisolone). Tocopherol acetate should not be taken with iron or silver preparations; with agents that exhibit an alkaline reaction (sodium bicarbonate, trometamol), indirect anticoagulants (dicoumarin, neodicoumarin). Pyridoxine may weaken the effect of levodopa and reduce toxic effects when using isoniazid and other anti-tuberculosis drugs. Pyridoxine hydrochloride enhances the effect of diuretics. Isoniazid, penicillamine, pyrazinamide, hydralazine, immunosuppressants, estrogens (including oral contraceptives containing estrogens) act as pyridoxine antagonists or enhance the renal excretion of the latter. Riboflavin is incompatible with streptomycin and reduces the effectiveness of antibacterial drugs (oxytetracycline, doxycycline, erythromycin, tetracycline, lincomycin). Tricyclic antidepressants, imipramine and amitriptyline inhibit riboflavin metabolism. Thiamine may weaken the curare-like effect. Antacids slow down the absorption of thiamine. The action of thiamine is inactivated by 5-fluorouracil, since the latter competitively inhibits the phosphorylation of thiamine to thiamine pyrophosphate.
