Essliver Forte
Essliver Forte is a hepatoprotector. It is used for fat metabolism disorders of various etiologies, steatosis and cirrhosis of the liver, toxic (alcohol, drugs, drugs) damage to this organ, psoriasis (as part of complex treatment). The effect of the drug is determined by the properties of each of its active ingredients. Essential phospholipids change the permeability of membranes of hepatocytes and their organelles, regulate the activity of enzymes associated with the lipid bilayer of membranes, thereby ensuring the normal course of ATP synthesis from adenosine diphosphate and inorganic phosphate. Essliver Forte normalizes the synthesis of basic cellular phospholipids and general lipid metabolism in case of various damage to liver cells. The drug promotes the structural regeneration of hepatocytes, restoring their membranes and taking on the damaging effects instead of membrane lipids. In addition to essential phospholipids, the drug contains the following active components:
• thiamine. As a coenzyme, it takes part in glucose metabolism;
• riboflavin. Stimulates tissue respiration processes;
• pyridoxine. As a coenzyme, it participates in protein metabolism;
• cyanocobalamin. Along with folic acid, it takes part in the biosynthesis of purine and pyrimidine nucleotides;
• nicotinamide. An indispensable participant in the processes of tissue respiration, lipid and glucose metabolism;
• tocopherol.
Pronounced antioxidant, performs a protective function in relation to polyunsaturated fatty acids in hepatocyte membranes from oxidative degradation of lipids.
A single dose of Essliver Forte is 2 capsules. Frequency of administration – 2-3 times a day. The duration of the medication course is three months or longer. It is possible to conduct a repeated course in agreement with the doctor. For psoriasis, the drug is taken for two weeks as an adjuvant. The optimal time to take it is with food. Capsules must be taken whole, without violating the integrity of the shell. Essliver Forte is well tolerated by patients. Side effects are very rare and may include dyspeptic disorders and allergic reactions. A contraindication to the use of the drug is individual intolerance to one or more components. Pregnancy and breastfeeding are not contraindications to taking the drug, but pharmacotherapy during this period should be carried out with extreme caution and under medical supervision. The drug can be used in pediatric practice. Its effectiveness has been confirmed in a number of clinical trials. It is one of the leading drugs in the Russian pharmaceutical product portfolio.

Who needs Essentiale® Forte N
Unfortunately, the liver usually does not give any specific signals about problems, therefore, to identify disorders, diagnostics are necessary (for example, ultrasound of the liver, biochemical blood test)10. Essential phospholipids are used for non-alcoholic fatty liver disease (risk factors for the development of which are usually overeating and lack of physical activity), alcoholic liver disease, as well as other liver pathologies10, 12. The active ingredient Essentiale® Forte N - essential phospholipids - as times reduces the severity of fatty infiltration3, 6, 7.
Indications for use of Essentiale® Forte N:
- Fatty liver disease
- Chronic viral hepatitis
- Alcoholic fatty liver disease
Contraindications to the use of Essentiale® Forte N:
- hypersensitivity to phosphatidylcholine, the main essential phospholipid in the drug;
- hypersensitivity to soy, soybeans (the drug contains soybean oil, so it is important to make sure there is no allergic reaction) and other components of the drug4;
- age up to 12 years (and weight

Description of the dosage form
Hard gelatin capsules with a red body and a brown cap, containing powder from pinkish-yellow to orange, the presence of particles in the form of a compacted powdery mass from yellowish-white to brown is allowed.
Characteristic
Combined drug.
Directions for use and doses
Orally during meals, swallowing whole with sufficient amount of water.
Adults: 2 capsules 2-3 times a day.
Children from 12 to 18 years old - 1 capsule 2-3 times a day.
The duration of the course of treatment is 3 months; if the course of treatment is extended, consultation with a doctor is required.
For psoriasis, take 2 capsules 3 times a day for 2 weeks.
Pharmacodynamics
The combined drug has hepatoprotective, hypolipidemic and hypoglycemic effects. Regulates the permeability of biomembranes, the activity of membrane-bound enzymes, ensuring the physiological norm of oxidative phosphorylation processes in cellular metabolism. Restores hepatocyte membranes through structural regeneration and the method of competitive inhibition of peroxide processes. Unsaturated fatty acids, being embedded in biomembranes, take on toxicogenic effects instead of membrane lipids of the liver, and normalize liver function, increasing its detoxification role.

Pharmacological properties of the drug Essliver forte
Pharmacodynamics. The main active component of the drug is natural phospholipids (substance EFL-NABROS), which are diglyceride esters of choline phosphoric acid of natural origin with a predominance of polyunsaturated fatty acids, mainly linoleic (70%), linolenic and oleic. Phospholipids are the main element of the structure of the cell membrane and cellular organelles, which affect the permeability of the cell membrane. They support the functions of the cell membrane: ion exchange, intracellular respiration, biological oxidation, influence the binding of intracellular respiration enzymes in mitochondria and oxidative phosphorylation in the process of cell energy metabolism. In healthy people, hepatocytes contain a sufficient amount of phospholipids; their content decreases when the liver is damaged, which leads to dysfunction of the cell membrane. Due to phospholipid deficiency, fat metabolism is impaired, which in turn leads to fatty liver degeneration. The use of Essliver Forte normalizes liver function and enzyme activity by directly influencing the structure of hepatocyte membranes and normalizes the energy balance of liver cells. The vitamins included in the preparation act as coenzymes in various biochemical enzymatic processes. Pharmacokinetics. When taken orally, more than 90% of the drug is absorbed in the small intestine. The main amount, under the influence of phospholipase-A, breaks down to 1-acyl-lyso-phosphatidylcholine, 50% of which is immediately reacytylated to polyunsaturated phosphatidylcholine during absorption in the small intestine. Polyunsaturated phosphatidylcholine enters the blood through the lymphatic vessels, mainly in combination with HDL and is transported to the liver. The maximum level of phosphatidylcholine in the blood serum is reached 6–24 hours after taking the drug and averages 20%. The half-life for the choline component is 66 hours, for saturated fatty acids - 32 hours. About 5% of the drug is excreted in the feces.
