pharmachologic effect
Multivitamin preparation. The pharmacological effect of the drug is determined by the active substances included in its composition. The drug regulates metabolic processes and normalizes metabolism. Vitamin A (retinol) contributes to the proper functioning of the body, plays an important role in normal tissue regeneration, synthesis of proteins, lipids, enzymes (lipase, trypsin). Necessary for the formation of bones, epithelial cells (skin, mucous membranes of the eyes, respiratory, digestive and urinary tracts). Retinol is a necessary component for the normal function of the retina of the eye, is involved in the processes of photoreception (shadow adaptation), and has a positive effect on the function of the lacrimal, sebaceous and sweat glands. Necessary for normal gonadal function and embryonic development of the fetus; for interaction, division of immunocompetent cells, normal synthesis of immunoglobulins and other factors of protection against infections. Provides a high level of antioxidant protection for the body. Vitamin Bi (thiamine hydrochloride) is the most important coenzyme in the metabolism of carbohydrates and is necessary for the normal functioning of the central and peripheral nervous systems, cardiovascular and endocrine systems. Vitamin B2 (riboflavin) is a component of enzymes that play an important role in redox metabolic reactions, cellular respiration and visual perception (color, light, dark adaptation), takes part in the formation of DNA, hemoglobin synthesis, and tissue regeneration. Vitamin B2 is necessary for the normal functioning of the liver, hematopoiesis, fetal development during pregnancy and for the growth of children. Vitamin C (ascorbic acid) takes part in many redox processes in the body, hemoglobin synthesis, tissue regeneration, collagen synthesis, maintaining the colloidal state of the intercellular substance, affects amino acid metabolism, accelerates the absorption of iron from the gastrointestinal tract, increases nonspecific resistance of the body, activates synthesis antibodies, interferon, phagocytosis. Vitamin C is necessary for the growth and formation of bones, skin, teeth and for the normal functioning of the nervous and immune systems.
Indications for use
A prophylactic remedy for children and adults in conditions accompanied by an increased need for vitamins (period of intensive growth in children; increased unbalanced and poor nutrition; during the recovery period after illnesses and to increase the body's resistance to infectious and colds.
Directions for use and doses
Orally, 10-15 minutes after meals daily for 1-2 months. For preventive purposes, adults take 1 tablet 2 times a day; pregnant women - 1 tablet per day in the first trimester, but no more than 2 tablets in the II-III trimesters; breastfeeding women - 1 tablet 1-2 times a day; children over 6 years old - 1 tablet per day (use in children under 6 years of age is contraindicated due to the dosage form). For therapeutic purposes: Adults and children over 14 years of age take 1 tablet 3 times a day; children from 6 to 10 years old - 1 tablet 2 times a day, from 11 to 14 years - 1 tablet 3 times a day. The duration of treatment depends on the nature of the disease. Doses of the drug can be increased depending on the body's need for vitamins. The duration of use for therapeutic purposes is determined by the doctor individually and can be 1-2 months. The dose of the drug can be adjusted depending on the body's need for vitamins. Special groups of patients There is no need to adjust the dose of the drug in the elderly or in patients with renal failure. Use with caution for therapeutic purposes in patients with liver failure.
Use during pregnancy and lactation
Use during pregnancy and breastfeeding is possible only for prophylactic purposes, subject to the recommended doses. Women who took high doses of retinol (more than 10,000 IU) can plan pregnancy no earlier than 6-12 months later. This is due to the fact that during this time there is a risk of improper development of the fetus under the influence of high levels of vitamin A in the body.
Precautionary measures
The drug is not recommended to be prescribed together with other multivitamins, since an overdose of the latter in the body is possible. The presence of vitamin A in the composition of the drug should be taken into account when used together with other products containing this vitamin. Patients with rare hereditary problems of fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase deficiency should not take this medicine. Patients with diabetes mellitus should take into account that each tablet contains 0.405 g of sugar and adjust the dose of hypoglycemic medications. Increased intake of ascorbic acid over a long period may result in increased renal clearance of ascorbic acid. Ascorbic acid as a reducing agent can distort the results of various laboratory tests (blood glucose, bilirubin, transaminase activity, LDH, glucose in urine, including causing false-negative results of fecal occult blood tests). Prescribed with caution to patients with acute nephritis, cholelithiasis, glucose-6-phosphate dehydrogenase deficiency, chronic pancreatitis, idiosyncrasy, hemochromatosis, thalassemia, sideroblastic anemia. Not recommended for patients with celiac disease due to the presence of wheat flour in the composition.
Interaction with other drugs
At the recommended prophylactic dose, no clinically significant interactions have been reported. Vitamin A (at a therapeutic dose, with a daily intake of 33,000-200,000 IU, 13-80 tablets) Neomycin - reduces the absorption of vitamin A. Retinoids - risk of hypervitaminosis of vitamin A. Oral contraceptives - can increase plasma levels of vitamin A. Cholestyramine, cholestipol - impaired absorption of vitamin A. Salicylates and glucocorticoids - reducing the risk of side effects of drugs. Thiamine hydrochloride Thiamine hydrochloride, influencing polarization processes in the area of neuromuscular synapses, can weaken the curare-like effect. Riboflavin Alcohol - difficulty in absorption. Probenecid, tricyclic antidepressants, phenothiazines - reduce absorption (require increasing the dose of riboflavin). Tricyclic antidepressants, imipramine and amitriptyline, inhibit the metabolism of riboflavin, especially in cardiac tissue. Riboflavin is incompatible with streptomycin and reduces the effectiveness of antibacterial drugs. Riboflavin in large doses can cause yellowing of urine and affect the diagnostic results of determining catecholamines and urobilinogen in the urine. Ascorbic acid Ascorbic acid increases the renal excretion of amphetamine. Ascorbic acid increases iron absorption. Ascorbic acid increases the absorption of aluminum hydroxide when administered together in patients with normal renal function; in renal failure, the dose of aluminum can reach toxic levels. Smoking, oral contraceptives - reduce the concentration of ascorbic acid in the blood plasma. Deferoxamine - increased toxicity of tissue iron (decompensation of cardiac activity). Separate administration by 1-2 hours and should not be given within the first month after starting deferoxamine treatment. Salicylates, short-acting sulfonamides - risk of developing crystalluria. Indirect anticoagulants (coumarin and indanedione derivatives), heparin - their effectiveness decreases with a dose of ascorbic acid of 10 g or more.
Contraindications
Hypersensitivity to the components of the drug; children under 6 years of age, nephrolithiasis, hypervitaminosis A, chronic glomerulonephritis, history of sarcoidosis, impaired iron or copper metabolism, hyperoxaluria.
Compound
active ingredients: 1 dragee contains retinol acetate (vitamin A) oil solution, in terms of 100% retinol acetate - 0.86 mg (2500 IU) or retinol palmitate (vitamin A) oil solution, in terms of 100% retinol palmitate - 1 .38 mg (2500 IU), thiamine hydrochloride (vitamin B1) in terms of 100% anhydrous substance - 1 mg, riboflavin (vitamin B2), in terms of 100% and dry matter - 1 mg, ascorbic acid (vitamin C), in terms of 100% substance - 35 mg;
Overdose
Fat-soluble vitamins can accumulate in tissues. In overdose they are more toxic than water-soluble ones. Symptoms of acute overdose of vitamin A (develop 6 hours after taking a dose of 300,000 units to 1 million units (120-400 tablets) once): drowsiness, lethargy, diplopia, dizziness, severe headache, nausea, severe vomiting, diarrhea, irritability, osteoporosis, bleeding from the gums, dryness and ulceration of the oral mucosa, peeling of the lips, skin (especially the palms), agitation, confusion. The clinic is reversible. Large doses of ascorbic acid can cause diarrhea and the formation of oxalate kidney stones. Water-soluble vitamins (B, Br, C), when they enter the body in excess, being highly soluble in water, are quickly eliminated from the body. Treatment: drug withdrawal; symptomatic therapy.
Side effect
The drug is usually well tolerated; in isolated cases, adverse reactions are possible. From the immune system: in persons with hypersensitivity, allergic reactions are possible, including anaphylactic shock, angioedema, hyperthermia; rarely - bronchospasm in persons with hypersensitivity to vitamins A, C, group B. From the skin and subcutaneous tissue: skin rashes, urticaria, itching, redness of the skin. From the gastrointestinal tract: dyspeptic disorders, nausea, vomiting, diarrhea, cramps. From the nervous system: headache, dizziness, increased excitability, drowsiness, sweating, peripheral neuropathy (with long-term administration of large doses). Others: urine may turn yellow, visual disturbances may occur. With long-term use in high doses, irritation of the mucous membrane of the gastrointestinal tract, arrhythmias, paresthesia, hyperuricemia, decreased glucose tolerance, hyperglycemia, renal dysfunction, dryness and cracks on the palms and soles, hair loss, seborrheic rashes may occur. If these adverse reactions occur, as well as reactions not mentioned in the package insert, you should consult a doctor.
