Pharmacodynamics and pharmacokinetics
Pharmacodynamics
Naftifine is a topical antimycotic that belongs to the class of allylamines. The mechanism of its action is associated with the ability to suppress the activity of the enzyme squalene-2,3-epoxidase, and thus inhibit the biosynthesis of ergosterol, which is part of the cell wall of the fungus.
The activity of the drug is manifested in relation to:
- dermatophytes (microsporum, epidermophyton, trichophyton);
- yeast fungi (Pityrosporum, Candida spp.)
- molds (Aspergillus spp.)
- other fungi (for example Sporothrix schenckii).
Naftifine against aspergillus and , and against yeast fungi - depending on the strain of the microorganism, it can exhibit both fungicidal and fungistatic activity.
It has antibacterial activity against Gram (+) and Gram (-) bacteria that can cause secondary infection , as well as an anti-inflammatory effect (which allows you to quickly eliminate the symptoms of inflammation, especially itching).
Pharmacokinetics
After application to the skin/nail, it penetrates well into their various layers and creates stable antifungal concentrations in them. naftifine is used externally, no more than 6% of the substance is exposed to systemic absorption.
The absorbed amount is partially metabolized and excreted in the urine and intestinal contents. T1/2 - 2-3 days.
Exoderil cream for external use 1% 30g
Compound
Active substance: naftifine hydrochloride - 10 mg.
Excipients: sodium hydroxide - 1.2 mg, benzyl alcohol - 10.0 mg, sorbitan stearate - 19.0 mg, cetyl palmitate - 20.0 mg, cetyl alcohol - 40.0 mg, stearyl alcohol - 40.0 mg, polysorbate 60 - 61.0 mg, isopropyl myristate - 80.0 mg, water purified - 718.8 mg.
Pharmacokinetics
When used externally, it penetrates well into various layers of the skin, creating stable antifungal concentrations in its various layers. After application to the skin, less than 6% of naftifine is systemically absorbed. The absorbed amount is partially metabolized and excreted by the kidneys and intestines. The half-life of the drug is 2-3 days.
Indications for use

Pharmacodynamics
Naftifine is an antifungal agent for external use, belonging to the class of allylamines. The mechanism of action is associated with inhibition of squalene-2,3-epoxidase, which leads to a decrease in the formation of ergosterol, which is part of the cell wall of the fungus. Active against dermatophytes such as Trichophyton, Epidermophyton and Microsporum, molds (Aspergillus spp.), yeasts (Candida spp., Pityrosporum) and other fungi (for example, Sporothrix schenckii). Naftifine has a fungicidal effect against dermatophytes and aspergillus. Against yeast fungi, the drug exhibits fungicidal or fungistatic activity depending on the strain of the microorganism. It has antibacterial activity against gram-positive and gram-negative microorganisms that can cause secondary bacterial infections. Instructions for use of Exoderil (Method and dosage)
Cream Exoderil, instructions for use
When the fungal process is localized on the skin, it is recommended to apply the ointment to the affected area 1 r./day, covering the adjacent areas of healthy skin (approximately 1 cm at the edges of the affected area).
Before applying the cream, the affected area should be thoroughly cleaned and dried.
The duration of treatment for dermatomycosis is usually from 2 to 4 weeks, but if necessary it can be extended to 8 weeks, for candidiasis - 4 weeks.
For nail fungus, apply the cream 2 times a day. Recommendations on how to use the drug for onychomycosis indicate that before the first application of the product, use a nail file or scissors to remove as much of the affected part of the nail as possible. Treatment is long-term – up to six months.
Exoderil solution: instructions for use
For skin mycoses , Exoderil drops are used 1 r./day, for onychomycosis - 2 r./day. The affected area of the skin should be thoroughly washed and dried, the affected area of the nail should be removed as much as possible using a nail file or scissors (to facilitate the procedure, on the recommendation of a doctor, it is allowed to use special emollients).
The duration of therapy for dermatomycosis is up to 4-8 weeks, for fungal infections of the nails - up to six months.
For otomycosis, treatment should last at least 14 days. Treatment is carried out by placing turundas soaked in a solution into the ear. The duration of exposure is 5-8 minutes. You need to repeat the procedure 1-2 times a day.
Additionally
To prevent relapse, Exoderil (both cream and solution) should continue to be used for at least 2 weeks after the clinical symptoms of the disease have resolved.
How to use Exoderil cream
If a fungus is detected on the skin, the ointment can be used on the problem area once a day. In addition to the affected area, the adjacent part of healthy skin needs to be treated.
Before applying, clean the area thoroughly and allow it to dry completely. The duration of the therapeutic course is on average 15-28 days, depending on the problem and severity of the condition.
For nail fungus, the cream can be applied 2 times a day. Before use, you need to remove the affected part of the nail plate with scissors and a file.
Treatment may take a long time - 4-6 months.
special instructions
Drops and cream are not intended for use in ophthalmology , and therefore contact with the eyes should be avoided.
The medicine does not have a negative effect on the ability to drive a vehicle/mechanics.
What is better - cream or solution?
A special feature of the drug Exoderil is that in addition to the antimycotic effect , it also has antibacterial activity : the drug affects pathogenic microorganisms, often combined with fungal infections.
The solution penetrates well into the nail, being evenly distributed in it and helping to destroy the fungus, relieving inflammation and eliminating itching.
For some diseases (in particular, mycoses of the feet , when both the nail plate and the skin are simultaneously affected), both dosage forms are often used in combination: the nail is treated with a solution in the morning, and cream is applied to the skin in the evening.
In severe cases, in addition to topical therapy, systemic antimycotics (tablets, solution for infusion, powder for the preparation of oral suspension) may be prescribed.

Analogs are cheaper than Exoderil
Cheap analogues of Exoderil are domestically produced drugs. Analogues produced by Russian companies may contain the same active ingredient, but their price will be lower in comparison with foreign products.
The first and only substitute for Exoderil in Russia is the drug Mikoderil . A one percent solution of Mikoderil can be purchased from 380 rubles, cream - from 290 rubles.
The price of Russian analogues with a similar mechanism of action is from 60 rubles.
Which is better: Loceril or Exoderil?
Loceryl is an antifungal nail polish whose active ingredient is amorolfine .
Amorolfine is used for fungal nail infections caused by Epidermophyton, Trichophyton, Mycrosporum, Coccidoides, Cryptococcus, Wangiella, Pityrosporum, Candida, Hendersonula, Histoplasma, Sporothrix, Alternaria, Cladosporium, Fonseceae, Scopulariopsis.
naftifine it contains .
Since the drugs differ in composition, it is not entirely correct to judge which is more effective - Exoderil or Loceril .
Loceryl is used to treat nail fungus , its analogue can be used both for the treatment of nail fungus and for the treatment of dermatomycosis . Moreover, the solution penetrates the nail plate better than varnish and acts the same regardless of whether the structure of the nail is damaged or not.
The disadvantages of nail polishes are their inability to penetrate into the channels of the nail plate, as well as into loose and crumbled nail plates.
Reviews allow us to conclude that Exoderil is quite effective against infections caused by the most common fungi today, however, it is powerless against some organisms that its analogue can easily cope with.
Thus, the choice in favor of one or another remedy must be made taking into account the diagnosis, pathogen and degree of damage.
Mikozan or Exoderil - which is better?
The drug Mikozan is a kit for removing fungal infections from the nail. The product is available in the form of a serum and is supplied with disposable nail files and an applicator brush, which makes it easier to apply the medicine to the affected nail.
The active substance of Mycozan is rye enzyme filtrate. When applied to the nail, the drug has a local antifungal effect , while increasing the density of the nail plate, reducing its porosity and creating conditions unfavorable for the growth of pathogenic fungi.
Experts often recommend using Mikozan for prevention, however, patients who have used this drug for treatment speak of it as an effective and really helpful remedy when it comes to onychomycosis .
If the affected area is not limited to the nail, then it is more advisable to use Exoderil to treat adjacent areas of the skin.
Use during pregnancy
Provided that the drug is used in accordance with the recommendations given in the instructions, its effect on the fetus/newborn is unlikely.
The results of teratogenicity studies indicate that there is no risk of any embryotoxic effects.
During pregnancy and breastfeeding, the medicine can be used only after a thorough assessment by the doctor of the expected effect of therapy/risk ratio.
Breastfeeding women should avoid getting the solution/cream on the skin and in the baby's digestive tract.
Contraindications and adverse reactions
You should avoid using Exoderil in the following cases:
- the presence of individual intolerance to the constituent components;
- breastfeeding period;
- pregnancy.
Children should take it with great caution.
In some cases, when treating skin and nail fungus, excessive dry skin and a burning sensation may be observed. Adverse reactions are reversible, so there is no need to discontinue treatment.
No cases of overdose have been identified to date.
Reviews about Exoderil
Reviews about Exoderil cream, as well as reviews about Exoderil solution, can be found on forums that are different: for some, this drug helps restore the health of their nails and skin, for others, on the contrary, even after 2-3 months of using the product, there is no improvement notices.
Experts say that you can get rid of nail/skin fungus only if the treatment is carried out correctly.
Since in case of fungal infections of the feet, the pathological process in about half of the cases spreads to the nails, it is considered advisable to combine ointment and drops: for example, the solution for nail fungus is used in the morning, and at night the skin and affected nail plates are treated with cream.
Any external remedies (drops, cream or varnish) against nail fungus should be used regularly and over a long period of time.
At the same time, it is very important (especially during the treatment period) to properly care for your nails/skin (for example, once a week, give your nails a bath with soda and laundry soap), do not forget about personal hygiene and the need to disinfect all surfaces in contact with the fungus.
In severe forms of fungal infection, topical antimycotic drugs (Exoderil or its analogues) are combined with systemic drugs to achieve a positive effect.
Which is better: Exoderil or Lotseril
Loceryl is a nail polish that has an antifungal effect. The active ingredient is amorolfine. This substance is used for nail fungus caused by various harmful microorganisms.
Since both medications have different components in their composition, it is not entirely appropriate to compare their effectiveness.
Loceryl is prescribed for fungal infections of the nail plate. The solution, compared to varnish, is able to penetrate much deeper into the nail and has the same effect, regardless of the degree of damage to the nail. Varnishes, in turn, cannot penetrate the channels of the nail plate.
