A combined drug with anti-inflammatory and antibacterial effects.

Fluocinolone acetonide is a synthetic corticosteroid for external use. It is 40 times more active than hydrocortisone. It has anti-inflammatory, antipruritic, antiexudative, antiallergic and vasoconstrictive effects. The mechanism of anti-inflammatory action is not fully understood. It is believed that fluocinolone acetonide reduces the manifestations of the inflammatory process by inhibiting the synthesis of prostaglandins and leukotrienes by inhibiting the activity of phospholipase A2 and reducing the release of arachidonic acid from cell membrane phospholipids. As a result, hyperemia and exudation in the lesion are reduced. Stabilizes cellular and subcellular (including lysosomal) membranes. Reduces the release of proteolytic enzymes from lysosomes. Reduces membrane permeability, mitotic activity and the release of inflammatory mediators. When exposed to the skin, the marginal accumulation of neutrophils is prevented, which leads to a decrease in inflammatory exudate and the production of lymphokines, inhibition of macrophage migration, and a decrease in the processes of infiltration and granulation. Inhibits the alteration phase and limits the focus of inflammation, reduces the migration of leukocytes and lymphocytes to the focus of inflammation. Inhibits the synthesis or release of cytokines (interleukins and interferon) from lymphocytes and macrophages. Reduces the severity of the early immune response.

Neomycin is an antibiotic of the aminoglycoside group. In low concentrations it has a bacteriostatic effect, in high concentrations it has a bactericidal effect. The mechanism of action is associated with a direct effect on ribosomes and inhibition of protein synthesis in the bacterial cell.

Active against gram-positive microorganisms:

Staphylococcus spp., Streptococcus spp., Corynebacterium diphtheriae, Listeria monocytogenes, Bacillus anthracis, Clostridium spp., Actinomyces spp., Neisseria meningitidis;
gram-negative microorganisms:
Escherichia coli, Shigella spp., Salmonella spp., Proteus spp., Enterobacter aerogenes, Vibrio cholerae, Haemophilus influenzae, Klebsiella pneumoniae, Bordetella pertussis, Pasteurella multocida, Borrelia spp., Treponema pallidum, Fusobacterium spp., Leptospira spp.

Has activity against acid-fast Mycobacterium tuberculosis.

Resistance of microorganisms to neomycin develops slowly and to a small extent.

As a result of the combined effect of fluocinolone acetonide and neomycin, the drug suppresses the development of inflammatory and allergic skin reactions complicated by bacterial infection.

With long-term use and/or application to large surfaces of the skin, wounds, damaged skin, the use of large doses, when using occlusive dressings and when treating children with Flucinar N ointment, systemic absorption of the glucocorticosteroid is possible; it is possible to suppress the function of the hypothalamic-pituitary-adrenal system and develop symptoms of hypercortisolism; Bacterial resistance to neomycin sulfate may also develop.

During drug therapy, periodic monitoring of the function of the adrenal cortex is necessary by determining the level of cortisol in the blood and urine after stimulation of the adrenal glands with adrenocorticotropic hormone (ACTH).

In case of development of a secondary bacterial or fungal infection, it is necessary to prescribe local use of an antibacterial or antifungal drug. If symptoms of infection persist, you should stop using Flucinar N until the infection has resolved.

Flucinar N should be used with caution on the skin around the eyes or eyelids due to the risk of developing glaucoma or cataracts, as well as in patients with a history of these diseases, who may experience an exacerbation of these diseases.

If a patient experiences symptoms such as blurred vision or other visual disturbances, consider referring the patient to an ophthalmologist to investigate possible causes of the disturbance, which may include cataracts, glaucoma, or rare diseases such as central serous chorioretinopathy.

Flucinar N should be used with extreme caution on the skin of the face and intertriginous skin (skin folds, armpits, groin, folds of the arms and legs), due to increased absorption of glucorticosteroids through thin skin and the possibility of side effects (telangiectasia, skin atrophy, perioral dermatitis) even after short-term use.

The drug should be used with caution in case of atrophic changes in the subcutaneous tissue, especially in the elderly.

Flucinar N should be used with extreme caution in patients with psoriasis, due to the possible relapse of the disease caused by the development of tolerance, the risk of generalized pustular psoriasis and general toxicity due to pressure ulcers.

Given the presence of an aminoglycoside antibiotic in the composition, Flucinar N is used with caution in cases of renal failure, since neomycin can accumulate in the kidneys. Cross-allergy to aminoglycoside antibiotics is possible.

Due to the ototoxic and nephrotoxic effects of neomycin, use of Flucinar N over large areas of skin, on broken skin, or over a long period of time may cause hearing impairment, including hearing loss and impaired renal function.

The drug Flucinar N is used with caution in patients with impaired renal function and hearing impairment. The risk of nephrotoxicity and ototoxicity is higher in patients with renal dysfunction. A cross-allergic reaction to aminoglycosides may occur.

When Flucinar N ointment is used together with other ototoxic and nephrotoxic drugs, their effect may be enhanced.

Long-term use of the drug Flucinar N can lead to the emergence of neomycin-resistant strains of bacteria and allergic reactions.

Children and teenagers

Flucinar N ointment should be used with caution, especially as long-term therapy, in children over 2 years of age, since in this group of patients, due to a higher ratio of body surface area to body weight than in adults, hypothalamic dysfunction may develop more quickly - pituitary-adrenal system and undesirable effects characteristic of glucorticosteroids may occur, including growth and development delays.

Propylene glycol, which is part of the drug Flucinar N, can cause skin irritation, and lanolin can cause local skin reactions (for example, contact dermatitis).

Indications for use

Flucinar gel and ointment are used for external lesions. Therefore, the main indications for the use of ointment and gel are the treatment of severe inflammatory and allergic diseases of the skin and mucous membranes:

neurodermatitis;
seborrheic dermatitis;
eczema;
psoriasis;
lichen planus.

Contraindications to the use of the drug Flucinar

It is not recommended to use Flucinar ointment for bacterial, viral and fungal skin infections, acne and rosacea, perioral dermatitis (dermatitis perioralis), after preventive vaccinations, as well as in cases of confirmed hypersensitivity to fluocinolone acetonide, other corticosteroids or other components of the drug. Should not be used in children under 2 years of age. It is not recommended to use the drug during pregnancy, especially for a long time and on large areas of the skin. The drug should not be used in the first trimester of pregnancy.

Use of the drug Flucinar

Intended for topical use. The ointment should be applied in a thin layer to the affected skin 1-2 times a day. The ointment should not be used under an occlusive dressing. Only for psoriasis is it possible to use a closed bandage, which must be changed daily. Treatment should not be carried out continuously for more than 2 weeks. Do not use the ointment on the skin of the face for more than 1 week. During the week it is recommended to use no more than 15 g of ointment (1 tube). With caution, under the supervision of a physician, use the drug in children over 2 years of age, only once a day, on a small area of skin; Do not apply to facial skin.

Side effects of the drug Flucinar

The following side effects may occur: acne, post-steroid purpura, inhibition of epidermal growth, subcutaneous tissue atrophy, dry skin, excessive body hair growth or alopecia, depigmentation or hyperpigmentation of the skin, atrophy and loss of skin integrity, telangiectasias, perioral dermatitis, furunculosis, atrophic streaks (striae), secondary infections. In some cases, the following may appear: urticaria, itching, burning, contact dermatitis, pstantic-papular rash or exacerbation of existing skin changes. The use of ointment under an occlusive dressing, given the increased absorption, can cause a pronounced systemic effect, which manifests itself in the form of edema, arterial hypertension, and a decrease in the body's resistance to infections. When applied externally to the skin of the eyelids, it can sometimes contribute to the development of cataracts or glaucoma.
