What is ichthyol ?
The pharmacopoeia states that this substance is ammonium bituminous sulfonate salt , obtained from resins (derivatives of oil shale). In medicine, drugs based on this component began to be used back in the 80s of the 19th century for the treatment of gout, rheumatism , etc. Pure ichthyol is not used; ointments and suppositories are made on its basis for use in medical practice.
Medicines containing this component have local anesthetic, anti-inflammatory, antiseptic, and keratoplasty effects . Also, ointment and suppositories can improve blood circulation in the skin. The substance moderately irritates nerve endings, thereby reducing their sensitivity, leads to denaturation of protein molecules, and regulates normal vascular tone.
The drug is not absorbed into the systemic circulation and has only a local effect on the skin or mucous membrane.
Indications for use
What is Ichthyol ointment for?
The ointment has a fairly wide range of applications; it is prescribed for burns, neuralgia, arthritis, eczema, erysipelas, rheumatism, and for the treatment of acne.
The dosage form of the “suppository” is used for hemorrhoids and other inflammatory diseases of the pelvic organs.

For parametritis, prostatitis and metritis, suppositories are carefully inserted into the rectum. According to the instructions for use, ichthyol suppositories are prescribed in the amount of 1-2 pieces per day.

Pharmacological properties
Anti-inflammatory agent, has local analgesic and antiseptic effect. The anti-inflammatory effect is associated with inhibition of the release of inflammatory mediators. When administered rectally, the drug has an irritating effect on the mucous membrane of the rectum, as a result of which blood circulation in the pelvic organs reflexively increases, metabolism improves, which helps eliminate inflammation in the prostate gland in men, ovaries, fallopian tubes, and uterus in women.
Indications for use
Salpingoophoritis, endometritis, prostatitis as part of complex therapy.

Hypersensitivity to the components of the drug, children under 18 years of age.

Directions for use and doses

Among the indications for the active substance in Russia, neuralgia and arthritis are mentioned, but not in all instructions for the drug. Scientific laboratories in the West have studied the ability of ichthammol to treat osteoarthritis, bruises and sprains.
For neuralgia and arthritis, apply 10-20% ichthyol in an even layer to the affected area and rub into the skin. Then apply a bandage, which is changed after 1-3 days.
The external product Ichthammol 50% is produced abroad and is prescribed for joint sprains, bursitis and other orthopedic indications.
Ichthyol is used for exacerbation of hemorrhoids, eczema and dermatitis in the anus. The use of the product helps reduce pain and inflammation.

ICHTHYOL. Ichthyolum.Synonyms: bitaminol, ichthammol, ichthyol-ammonium, ichthyosulfol.
Properties.
A black, thin layer of brown syrup-like liquid with a peculiar pungent odor and taste. Soluble in water, glycerin, partially in alcohol and ether. Aqueous solutions foam when shaken.
Ichthyol contains at least 10.5% organically bound sulfur. It is a product of dry distillation of bituminous shale containing the remains of fossil marine animals and fish. Incompatible in solutions with alkaloids, salts of heavy metals, iodide salts, ammonia solutions, glycosides.
Store in glass jars.
Action and application.
It has an antiseptic, antiparasitic, anti-inflammatory, keratoplastic and local anesthetic effect. The antimicrobial and antiparasitic effect of ichthyol is explained by the content of sulfur and aromatic substances. After application to wounds and mucous membranes, in addition to the antiseptic effect, it constricts blood vessels, reduces secretion and exudation, moderates pain and accelerates the regeneration of affected tissue.
After intravenous administration, it acts as a nonspecific therapy - irritation, due to which a number of reflex reactions develop, accompanied by an increase in the body's defenses, an increase in the release of adrenaline, serotonin and vasopressin, and a constriction of blood vessels and capillaries.
Prescribed externally for various diseases of the skin (wounds, eczema, burns, dermatitis, furunculosis, pyoderma), subcutaneous tissue and muscles, for arthritis, neuralgia, tendonitis, tendovaginitis, bursitis, mastitis in the form of ointment (10-30%) or aqueous-alcoholic solutions (10-30%). Alcohol-ichthyol solutions (1:25) are widely used in the form of lotions for phlegmon and myositis. For skin diseases of parasitic origin, including scabies, scab, ringworm, it is used in the form of an ointment (10-20%), liniment and paste (5-10%). By adding tar, sulfur and creolin, the therapeutic effect for scabies and dermatomycosis increases.
As an antiseptic and anti-fermentation agent, it is used orally for acute rumen tympany, flatulence, functional intestinal disorder, inflammation of the stomach and intestines, and acute dilatation of the intestines in horses (colic). In case of acute dilatation of the stomach in horses, it is recommended to rinse the stomach with a 1-2% aqueous solution and give an alcohol-ichthyol-valerian mixture orally. Aqueous solutions of ichthyol (1-2%) are prescribed for prolapse and inflammation of the rectum (irrigation in the form of enemas), for lobar inflammation of the larynx, and inflammation of the esophagus. In obstetric practice, it is used for metritis, vaginitis, salpingitis in the form of douching with a 3-5% solution or suppositories (for small animals 0.1-0.3 g). For endometritis and subinvolution of the uterus in cows, ichthyol is used intramuscularly in the form of a 7% solution in a 40% glucose solution in a dose of 10 ml. The use of ichthyol 2-3 times a week helps to heal and restore the reproductive functions of the genital organs. An aqueous solution of ichthyol is used orally for coccidiosis in lambs once a day for 3 days in a row, then a three-day break, and again prescribed for three days.
Ichthyol is included in the composition of finished medicines: a) ichthyol-naphthalan paste (4 parts ichthyol, 10 parts zinc oxide, 16 parts naphthalan ointment and 10 parts starch), used for dermatomycosis and eczema; b) zinc-ichthyol paste (zinc oxide 10 parts, ichthyol 1, petroleum jelly 19 parts), prescribed for eczema, seborrheic skin lesions; c) ichthyol in glycerin (10%); d) ichthyol ointment (10% and 20%) and other drugs used for skin diseases. Intravenous sterile solutions of ichthyol (2%) are recommended to be administered as a hemostatic agent for colic (sand), acute dilatation of the stomach in horses, intestinal atony (horses 50-150 ml, small ruminants 10-20 ml). There are reports of the positive effect of ichthyol (0.25-0.5% solution) when lambs and sheep eat wool; lambs are injected with 30-90 ml of this solution, sheep 100-300 ml per day for 7 days in a row, then after 10 days the solution is given again for seven days.
A 3-5% solution of ichthyol is used to treat thelaziosis in cattle by washing the eyes. After removing the horns, the wound surface is tamponed with a paste consisting of 2 g of ichthyol, 10 g of wax or paraffin and 2 g of vaseline oil. Ichthyol is prescribed orally in the form of solutions (1-2%), boluses, mixtures, porridges.
