Miconazole is an antifungal agent that destroys dermatophytes and yeasts , but does not disturb the pH or destroy healthy microflora in the vagina. The substance directly affects the synthesis of lipid components in the fungal membrane, thereby destroying it. It does not penetrate into the blood and is not found in the kidneys and products of the excretory organs; 90% remains in the vagina. The maximum concentration is after 8 hours.
Metronidazole is an antibiotic . It inhibits the process of RNA and DNA of destructive bacteria by restoring the 5-nitro group by transport proteins inside the cell. After entering the vagina, it penetrates into the blood. Its maximum concentration in the blood (half of the administered dose) can be detected after 9 hours.
Interaction
The medicine is not combined with ethanol, disulfiram , non-depolarizing muscle relaxants, cimetidine, and lithium preparations .
When taking indirect anticoagulants , it is worth additionally monitoring the prothrombin index .

Metronidazole is a drug for the treatment of trichomoniasis for topical and oral use. Miconazole nitrate is an effective antifungal drug used to treat infections caused by dermatophytes and Candida; in addition, when applied topically, it has a pronounced bacteriostatic effect against a number of gram-positive bacteria. The purpose of topical use of this combination drug is the local treatment of trichomoniasis, as well as the prevention of vaginal mycosis, which often occurs after treatment with metronidazole. The drug can also be used for vaginal mycoses not associated with treatment with metronidazole.
Pharmacokinetics
When administered intravaginally, metronidazole and miconazole nitrate are only slightly absorbed. According to studies, the serum level of metronidazole is 0.2 μg/ml, the level of miconazole nitrate is 0.3 μg/ml. Metronidazole, when administered orally, is usually well absorbed and peak plasma concentrations are achieved between the first and third hours. With a single oral dose of 250 mg, a peak plasma concentration of 5 mcg/ml is achieved, determined by gas chromatography. The bioavailability of the drug when taken orally is almost 100%. In studies conducted in healthy volunteers and patients, metronidazole rapidly penetrates the cerebrospinal fluid and reaches therapeutic concentrations in brain abscesses and lung abscesses. It has a significant volume of distribution and less than 20% of circulating metronidazole is bound to plasma proteins. The drug enters the biliary tract, where concentrations comparable to plasma levels are achieved. The average half-life of metronidazole in healthy volunteers is 8 hours. The main route of excretion of metronidazole and its metabolites is urine (60-80% of the dose), 6-15% of the dose is excreted in the feces.

Directions for use and doses
For trichomoniasis: simultaneously with taking metronidazole orally, 1 vaginal tablet Klion-D 100, slightly moistened beforehand, must be inserted deep into the vagina once a day (in the evening before bed) for 10 days. During these 10 days, you must take 2 metronidazole tablets (250 mg 2 times) daily orally (one tablet in the morning and one in the evening) during or after meals, without chewing. It is advisable to simultaneously treat the sexual partner with metronidazole tablets for oral administration. If treatment is ineffective: the 10-day course of treatment can be repeated. For candidiasis: 1 vaginal tablet Klion-D 100, slightly moistened beforehand, must be inserted deep into the vagina once a day (in the evening before bed) for 10 days.
Use during pregnancy and lactation
Pregnancy The drug is contraindicated in the first trimester of pregnancy. Metronidazole, when taken orally, penetrates the placental barrier and quickly enters the fetal bloodstream. Reproductive toxicity studies conducted in rats used doses up to five times higher than those used in humans. These studies showed no evidence of fertility impairment or fetal harm from metronidazole. When administered intraperitoneally to mice at a dose approximately corresponding to the dose used in humans, metronidazole had a fetotoxic effect. However, when administered orally to pregnant mice, no fetotoxic effect was observed. However, there are currently no adequate and well-controlled studies in pregnant women. Based on a meta-analysis of studies conducted in the first trimester of pregnancy, it was concluded that no increase in fetotoxicity was observed. Despite this, metronidazole can only be used during pregnancy (and especially in the first trimester of pregnancy) if the expected benefits outweigh the possible risks. Breastfeeding Metronidazole, when taken orally, is excreted into breast milk in concentrations similar to plasma concentrations. It can give breast milk a bitter taste. To protect the infant from the effects of the drug, it is necessary to stop taking metronidazole, or stop breastfeeding while taking metronidazole, as well as for 1 or 2 days after completion of therapy, taking into account the need for therapy for the mother.
Precautionary measures
During the course of treatment with Klion-D 100, as well as for at least one day after its completion, the consumption of alcoholic beverages is prohibited. During treatment with Klion-D 100, vaginal tablets, sexual intercourse should be avoided. If treatment is ineffective, it is recommended to replace it with another systemic trichomonadocidal and/or antifungal drug. If hypersensitivity or irritation of the mucous membranes occurs, treatment with this drug should be discontinued.
Interaction with other drugs
Data regarding the interaction of metronidazole and miconazole nitrate for vaginal use are not known to date. When using vaginal tablets Klion-D 100 in combination with metronidazole tablets for oral administration, the following types of drug interactions may occur: Metronidazole can potentiate the anticoagulant effect of oral anticoagulants, which can lead to an increase in prothrombin time, therefore, it is necessary to adjust their dose. Enzyme inducers (eg, phenytoin, phenobarbital) may accelerate the metabolism of metronidazole, which may result in decreased plasma levels and increased plasma clearance of phenytoin. Enzyme inhibitors (eg, cimetidine) may prolong the half-life and reduce the plasma clearance of metronidazole. Drinking alcoholic beverages while taking metronidazole may cause disulfiram-like side effects (intestinal colic, nausea, vomiting, headaches and facial flushing). Metronidazole and disulfiram should not be taken together (additive effects, psychotic reactions, confusion may occur) Taking metronidazole may increase the level of lithium in the blood plasma, and therefore it is necessary to reduce the dose or discontinue the lithium drug before starting metronidazole therapy. In cases of simultaneous use of cyclosporine and metronidazole, the concentrations of cyclosporine in the blood plasma may increase. If combination therapy with these drugs is necessary, it is recommended to monitor cyclosporine plasma concentrations. Metronidazole reduces the clearance of 5-fluorouracil and increases its toxicity. Metronidazole may interfere with certain serum biochemical parameters such as aspartate aminotransferase (AST), alanine aminotransferase (ALT), lactate dehydrogenase (LDH), triglycerides and glucose hexokinase.
Contraindications

Overdose
The drug is intended for intravaginal use only. In case of unintentional ingestion of a large amount of the drug, it is necessary to rinse the stomach. If symptoms of intoxication appear during an overdose (nausea, vomiting and ataxia), it is necessary to carry out symptomatic therapy, such as gastric lavage, activated charcoal, hemodialysis. There is no specific antidote. Metronidazole and its metabolites are readily excreted by dialysis.

The tablets are packaged in foil, it is difficult to get them out, the photo shows a cut that I made with scissors. The tablet is fragile and may crumble into several pieces when taken out of the foil.
The method of application seemed very convenient to me initially, nothing complicated: once a day (or rather at night), you should wet the tablet with water and insert it inside. It turned out, it seemed. The tablets are large (well, this is not a problem, you don’t need to swallow them), shaped like a drop. As soon as the water and the tablet come into contact, a chemical reaction begins: the tablets foam, become soft and literally melt in your hands. During this chemical reaction, you must manage to stuff the tablets where they should be. It’s not so easy to send a tablet to its destination; it tends to stay on your fingers.
The most unpleasant thing is that after administering the pill you need to sleep in one position and under no circumstances get up at night. The tablets strive to come out, but even overnight (6-7 hours of sleep) the tablets do not completely dissolve and in the morning they appear out in varying degrees of “wornness”. If you suddenly decide to use these tablets, then you should definitely use daily sanitary pads.
Okay, I’m ready to put up with the pill coming out, but it decided to finish me off by disrupting my cycle twice. Strictly, like clockwork, after using the third tablet, discharge began. After the first cycle failure, I went to a consultation with a complaint. The gynecologist said: this happens, everything will pass and continue treatment. After the second failure, I stopped taking this drug. I was cured with other pills that one of my friends recommended on this site. I went to the gynecologist only to get tested and get the coveted stamp of permission to work.
I have no desire to recommend these pills; I did not have time to feel the results of the treatment. It’s up to you to decide whether to buy these tablets or not, I just shared my opinion.

I chose Klion because it turned out to be the best price, plus a friend also used it and was very pleased. I can say the same because I was cured of both diseases in just 8 days. The symptoms did not recur, tests showed that the inflammation and fungus were eliminated, so in terms of effectiveness, I can give Klion an “excellent” rating. But regarding the convenience and comfort of treatment, everything is not so smooth. The tablets cause quite a strong burning sensation, especially in the first days. I introduced them at night, because of this, sometimes I lay awake until midnight and suffered from unpleasant sensations. I also didn’t like the fact that Klion leaks a lot. Even if you use it at night, foamy discharge occurs in a very large volume during the next day, and you cannot do without pads. The last unpleasant moment even scared me a little, because by the end of the treatment my urine began to resemble rust in color. Okay, after finishing the treatment it went away, but I still didn’t understand what it was connected with.

I went into the shower to douse myself with cold water and saw in the mirror that all the skin was red, with spots, if you touched, then in this place there were white marks from the touch, which then disappeared .. the entire sternum, part of the arms and cheeks were red, the cheeks were still and started to itch. In the end, I realized that the tachycardia was not accidental. I developed a severe allergy. and there were no antihistamines in the house... and you can’t run anywhere to get them at three o’clock in the morning. I had to wash everything out if possible and apply cold water. In the morning there was a slight tachycardia and the skin itched in places. The redness subsided. I think this is the end of my acquaintance with this drug and it will go in the trash. So be careful, and better yet, don’t get sick!
