For trichomoniasis
Orally,
adults
are prescribed a single dose of 250 mg 2 times a day in the morning and evening after meals for 10 days. Simultaneous treatment of sexual partners is carried out. Women are simultaneously prescribed vaginal tablets, 1 tablet. (500 mg) in the evening for 10 days.
For amebiasis
adults
are prescribed 250 mg 3 times a day.
The course of treatment is 5-10 days; For children,
the dose is set at the rate of 35-50 mg/kg body weight/day. The daily dose should be divided into 3 doses. The course of treatment is 10 days.
For giardiasis
Orally,
children over 10 years of age
are prescribed 500 mg/day (in 2 doses);
children aged 6-10 years
- 375 mg/day (in 3 doses);
2-5 years
- 250 mg/day (in 2 doses). The course of treatment is 5-10 days. If necessary, treatment can be repeated after 4-6 weeks.
In the treatment of infectious diseases caused by anaerobic bacteria,
Trichopolum can be used in monotherapy or in combination with other antibiotics.
The course of treatment is 7-10 days. For adults and children over 12 years of age,
the drug is prescribed 250-500 mg 3 times a day;
for newborns and children under 12 years of age
- at the rate of 7.5 mg/kg body weight 3 times a day. The drug should be taken during or after meals.
To prevent infections caused by anaerobic bacteria
(before surgical operations on the abdominal cavity, in obstetric and gynecological practice),

the drug is prescribed at a dose of 1 g, then 250 mg 3 times a day during or after meals until preoperative fasting;
children from 5 to 12 years old
are prescribed 125 mg every 8 hours for 2 days;

IV administration of Trichopolum is indicated for severe infection and in the absence of the possibility of taking the drug orally.
The solution for intravenous administration in 100 ml polyethylene bottles is ready for use. The solution that remains after administration is not reused. If there are visible changes in the solution, the drug should not be used.
Adults and children over 12 years old
the drug is prescribed intravenously every 8 hours, 100 ml of a 0.5% solution (500 mg). The infusion is carried out at a rate of 5 ml/min. The maximum daily dose for intravenous administration is 4 g. The course of infusion therapy is individual. In the future, depending on the severity of the infection, they switch to oral forms of Trichopolum.
Children under 12 years of age
The dose for intravenous administration is set at the rate of 7.5 mg/kg body weight every 8 hours. The drug is administered intravenously at a rate of 5 ml/min.
To prevent infections caused by anaerobic bacteria
(before planned surgery on the abdominal cavity and urinary tract),
adults and children over 12 years of age,
the drug is administered as an infusion at a dose of 0.5-1 g on the day of surgery and the next day at a dose of 1500 mg/day (500 mg every 8 hours ). After 1-2 days, they usually switch to oral maintenance therapy.

Treatment should not be interrupted during menstruation.
Trichopolum
The drug is prescribed orally, during or after meals, the tablets are taken without chewing.
Trichomoniasis
Adults and children over 10 years of age are prescribed 250 mg (1 tablet) 3 times a day or 500 mg (2 tablets) 2 times a day for 7 days. Women must additionally be prescribed metronidazole in the form of vaginal suppositories or tablets. If necessary, you can repeat the course of treatment. Bacterial vaginosis
Adults are prescribed 500 mg (2 tablets) 2 times/day for 7 days or 2000 mg (8 tablets)/day once. Simultaneous treatment of the sexual partner is not required.
Amoebiasis
For invasive forms of intestinal amebiasis in susceptible patients (including amoebic dysentery), adults and children over 10 years of age are prescribed 750 mg (3 tablets) 3 times a day. Children aged 3 to 7 years - 250 mg (1 tablet) 4 times a day; at the age of 7 to 10 years - 375 mg (1.5 tablets) 3 times a day. The course of treatment is usually 5 days.
For intestinal amebiasis in less susceptible patients and for chronic amoebic hepatitis, adults and children over 10 years of age are prescribed 375 mg (1.5 tablets) 3 times a day. Children aged 3 to 7 years - 125 mg (1/2 tablet) 4 times a day; at the age of 7 to 10 years - 250 mg (1 tablet) 3 times a day. The duration of the course of treatment is 5-10 days.
For amoebic liver abscess and other forms of extraintestinal amebiasis, adults and children over 10 years of age are prescribed 375 mg (1.5 tablets) 3 times a day. Children aged 3 to 7 years - 125 mg (1/2 tablet) 4 times a day; at the age of 7 to 10 years - 250 mg (1 tablet) 3 times a day. The course of treatment is 5 days.

Giardiasis

Children aged 3 to 7 years - 250-375 mg (1-1.5 tablets) 1 time / day for 5 days, or 500-750 mg (2-3 tablets) 1 time / day for 3 days. Children aged 7 to 10 years - 250 mg (1 tablet) 2 times/day for 5 days, or 1000 mg (4 tablets) 1 time/day for 3 days.
Periodontal infections
For acute ulcerative gingivitis, adults and children over 10 years of age are prescribed 250 mg (1 tablet) 3 times a day. Children aged 3 to 7 years - 125 mg (1/2 tablet) 2 times a day; at the age of 7 to 10 years - 125 mg (1 tablet) 3 times a day. The duration of treatment is 3 days.
For acute odontogenic infections, adults and children over 10 years of age are prescribed 250 mg (1 tablet) 3 times a day for 3-7 days.
Infections caused by anaerobic bacteria
Treatment of anaerobic infections usually begins with IV infusions. As soon as possible, treatment should be continued with metronidazole tablets.
Adults and children over 10 years of age are prescribed 500 mg (2 tablets) 3-4 times a day. The duration of treatment should not exceed 7 days.
For children under 10 years of age, the use of metronidazole in the form of infusion solutions is recommended for the treatment of anaerobic infections.
Helicobacter pylori eradication
Prescribe 500 mg (2 tablets) 3 times/day for 7 days (as part of combination therapy, for example, with amoxicillin 2.25 g/day).
Prevention of infections caused by anaerobic bacteria (before abdominal surgery, in gynecology and obstetrics)

Overdose
Symptoms: increased side effects, mainly nausea, vomiting and dizziness; in more severe cases, ataxia, paresthesia and convulsions are possible. The lethal dose for humans is unknown.
Treatment: symptomatic and supportive therapy. There is no specific antidote.

Directions for use and doses
For trichomonas vaginitis, the drug is prescribed 1 vaginal tablet per day for 7-10 days in combination with taking metronidazole tablets.
For nonspecific vaginitis and bacterial vaginosis, the drug is prescribed 1 vaginal tablet per day for 7 days, if necessary in combination with taking metronidazole tablets.

Pharmacodynamics
Antiprotozoal drug with antibacterial activity for topical use in gynecology. Refers to nitro-5-imidazoles. The mechanism of action of metronidazole is the biochemical reduction of the 5-nitro group of metronidazole by intracellular transport proteins of anaerobic microorganisms and protozoa. The reduced 5-nitro group of metronidazole interacts with the DNA of the cell of microorganisms, inhibiting the synthesis of their nucleic acids, which leads to the death of microorganisms.
Metronidazole is an effective antimicrobial and antiprotozoal agent with a broad spectrum of action. Shows high activity against Trichomonas vaginalis, Gardnerella vaginalis, Giardia intestinalis, Entamoeba histolytica, Lamblia spp., as well as against obligate anaerobes - Bacteroides spp. (Bacteroides fragilis, Bacteroides ovatus, Bacteroides distasonis, Bacteroides thetaiotaomicron, Bacteroides vulgatus), Fusobacterium spp., Veillonella spp., Prevotella spp. (Prevotella bivia, Prevotella buccae, Prevotella disiens) and some gram-positive microorganisms (Clostridium spp., Peptostreptococcus spp., Peptococcuc spp., Eubacterium spp., Mobiluncus spp.).
Pharmacokinetics

Metronidazole passes through the placenta, so the drug should not be prescribed in the first trimester of pregnancy. In the future, the drug should be used only if the potential benefit to the mother outweighs the possible risk to the fetus.
Metronidazole is excreted in breast milk. Breastfeeding should be discontinued during the period of use of the drug. Breastfeeding should be resumed no earlier than 48 hours after stopping the drug.
special instructions
It is recommended that the sexual partner be treated simultaneously with metronidazole, regardless of whether he or she has any manifestations of the disease.
During treatment with the drug, it is recommended to abstain from sexual intercourse.

Impact on the ability to drive vehicles and operate machinery

Drug interactions
The drug is compatible with sulfonamides and antibiotics.
Metronidazole causes ethanol intolerance, so alcohol should be avoided during treatment.
You should not combine metronidazole with disulfiram, since the interaction of these drugs may cause depression of consciousness and the development of mental disorders.
When used simultaneously with indirect anticoagulants (including warfarin), metronidazole enhances their effect, which leads to an increase in prothrombin time.
It is not recommended to use metronidazole in combination with non-depolarizing muscle relaxants (vecuronium bromide).
Under the influence of barbiturates, the effectiveness of metronidazole may decrease due to acceleration of its metabolism in the liver.
Cimetidine inhibits the metabolism of metronidazole, which can lead to an increase in its concentration in the blood plasma and an increased risk of adverse reactions.
When taken simultaneously with lithium preparations, it is possible to increase its concentration in the blood plasma.
