Xymelin 1 mg/ml 10 ml nasal drops.

Pharmacodynamics and pharmacokinetics

The main component of Xymelin is included in the group of local vasoconstrictors or decongestants that have an α-adrenomimetic effect. Its use causes a significant narrowing of the blood vessels in the area of the nasal mucosa, thereby eliminating swelling and hyperemia of the mucous membranes of the nasopharynx. Helps ease nasal breathing during the treatment of rhinitis . Therapeutic concentrations of the drug do not cause irritation of the mucous membrane and the development of hyperemia . The effect appears after a few minutes, lasting an average of 10-12 hours.

The drug is applied topically and is almost not absorbed. The composition of the plasma contains a completely insignificant content of the active substance.

Interactions of the drug Xymelin extra

The drug is incompatible with MAO inhibitors, tri- and tetracyclic antidepressants. Sympathomimetic drugs cause the release of catecholamines, including norepinephrine, which has a vasoconstrictor effect, resulting in an increase in blood pressure. If there is a significant increase in blood pressure, treatment with Xymelin Extra should be discontinued and symptomatic treatment administered. Concomitant use with tri- and tetracyclic antidepressants may increase the sympathomimetic effect of xylometazoline. With concomitant administration of other drugs with anticholinergic activity, the anticholinergic effect of the drug may be enhanced.

Indications for use

Xymelin drops or spray are recommended for use when:

  • allergic rhinitis in acute form;
  • acute respiratory diseases accompanied by rhinitis;
  • hay fever;
  • sinusitis;
  • otitis media;
  • preparing patients for diagnostic procedures in the nasal passages.

ATC code R01AA07 Pharmacological properties Pharmacokinetics When applied topically in recommended doses, the drug is practically not absorbed, plasma concentrations are so small that they cannot be determined by modern analytical methods. Pharmacodynamics Sympathomimetics with alpha-adrenergic activity for local use of the nasal mucosa. Xylometazoline has a vasoconstrictor effect, reducing swelling of the mucous membrane and promoting drainage of the nose and sinuses. There is a slight risk of effects on the heart and central nervous system. Long-term treatment can cause swelling of the mucous membrane and hypersecretion. The effect begins a few minutes after application and lasts up to 10 – 12 hours. Indications for use Symptomatic relief of nasal congestion caused by rhinitis and sinusitis: - acute respiratory diseases with symptoms of rhinitis (runny nose); - acute allergic rhinitis; - sinusitis; - hay fever. Directions for use and dosage Adults and children over 10 years of age: - Xymelin® 0.1%: 2 drops in each nasal passage Children from 2 to 10 years old: - Xymelin® 0.05%: 1-2 drops in each nasal passage The drug should not be used more than 3 times a day. The duration of treatment should not exceed 10 days. Side effects The most common adverse reactions are associated with the nasopharynx and mucous membranes. About 3-8% of patients experience the following adverse reactions. The frequency of side effects of the drug is assessed as follows: frequent (> 1/100,  1/10); uncommon (> 1/1000,  1/100); rare (>1/10,000,  1/1000); very rare ( 1/10,000). Often: - burning of the nasopharynx - irritation and dryness of the nasopharyngeal mucosa - sneezing - hypersecretion; Uncommon: - with frequent or prolonged use, swelling of the nasopharyngeal mucosa is possible and, as a result, impaired breathing through the nose; Rarely: - gastrointestinal discomfort; Very rare: - systemic allergic reactions - headache - anxiety - insomnia - fatigue - transient visual impairment - tachycardia or irregular pulse Contraindications: increased sensitivity to the components of the drug after surgery on the dura mater (during transsphenoidal hypophysectomy and other transnasal operations) closed-angle glaucoma in children age up to 2 years (for Xymelin® 0.05%) children up to 10 years of age (for Xymelin® 0.1%) Drug interactions When taken simultaneously with tri- and tetracyclic antidepressants, the systemic effect of xylometazoline and a possible increase in its sympathomimetic effect cannot be excluded. Xylometazoline should be used with caution and only after consultation with a physician in patients receiving mono-amine oxidase inhibitors (MAOIs) and other drugs that may increase blood pressure. Special instructions Xylometazoline 0.05% and 0.1% should not be given to children under 2 years of age due to the risk of overdose, which can lead to depression of the central nervous system. The duration of treatment should not be more than ten days, since long-term treatment with xylometazoline can cause swelling of the nasal mucosa and increased secretion due to increased sensitivity of cells - the “reverse effect”. Xylometazoline should be used with caution in persons sensitive to adrenergic substances, which may cause symptoms such as insomnia, dizziness, tremor, abnormal heart rhythm or high blood pressure. Xylometazoline should be used with caution and only after consultation with a doctor in patients with: hyperthyroidism, diabetes mellitus, hypertension, atherosclerosis, aneurysm, coronary heart disease, arrhythmia, tachycardia, pheochromocytosis, and in the presence of problems with urination due to an enlarged prostate. Pregnancy and lactation Xylometozoline should not be used during pregnancy due to possible systemic vasoconstrictor effects. Xylometazoline can be used during breastfeeding, provided that your doctor's recommendations are followed. Features of the effect of the drug on the ability to drive a vehicle or potentially dangerous mechanisms Xylometazoline does not affect the ability to drive vehicles or use equipment. Overdose Overdose due to accidental ingestion: Symptoms: CNS depression with a significant decrease in body temperature, drowsiness with sweating, headache, irregular pulse, potential coma in children, hypotension, hypertension. Overdose of Xymelin extra, symptoms and treatment

Excessive use of xylometazoline may cause dizziness, sweating, a sharp decrease in body temperature, headache, bradycardia, a significant increase in blood pressure, respiratory depression, and coma. Hypertension may give way to hypotension. Symptomatic therapy must be carried out under the supervision of a physician. With excessive use of ipratropium bromide, an overdose is unlikely due to the extremely insignificant absorption of the substance into the blood, but dry mouth, difficulty in accommodation, and tachycardia may develop. Treatment is symptomatic. A significant overdose can cause symptoms associated with the anticholinergic effect of the drug on the central nervous system, including hallucinations, for the elimination of which cholinesterase inhibitors are prescribed.

Reviews about Xymelin

It is generally accepted that nasal drops are the easiest auxiliary drug that can be used independently. However, reviews of Xymelin confirm the fact that such drugs can only be used as prescribed by a doctor.

Many patients talk about the emergence of real drug addiction. This usually happens during the season of colds and allergic reactions , when people begin to actively use such drugs and then cannot give them up due to addiction.

Those patients who used this drug in acceptable dosages and for the time determined by the doctor were divided into those for whom the treatment turned out to be quick and effective and those who did not notice much improvement.

The main thing is to use any treatment for rhinitis with basic caution. Each vasoconstrictor drug must be applied to clean and moistened mucous membranes. For this, simple saline solutions or special products are used, for example, Physiomer or Aquamaris . It is interesting that sometimes just these washings become enough for recovery and the need for medications simply disappears.
