Composition and release form of the drug

The release form of the drug "Ultraproct" is suppositories for rectal use and ointment for external use in the rectal area. Using these drugs together (both suppositories and ointment), it is possible to get rid of the unpleasant symptoms of hemorrhoids quite quickly, even at the second stage. Judging by the reviews of patients, thanks to the action of Ultraproct suppositories and ointment, they managed to avoid surgical intervention.

"Ultraproct" has antipruritic, antihistamine, local anesthetic and anti-inflammatory effects. The main active components of the ointment:

  • Fluocortolone is a glucocorticosteroid, i.e. a substance of hormonal origin. When applied locally, it prevents the accumulation of neutrophils at the edges, as a result, inflammation decreases, the synthesis of lymphokines slows down, itching and inflammation go away, local blood circulation accelerates, the migration of macrophages is normalized, and swelling subsides.
  • Cinchocaine is an anesthetic that is used for local external use. It blocks the sensory nerve endings that are located on the mucous membrane of the rectum and the outer rectal surface, as a result of which pain signals do not reach the brain and the patient does not experience pain.

As auxiliary components, Ultraproct ointment contains castor oil, 2-octyldodecanol; macrogol-400-monoricinoleate; lemon-pink scent.

Pharmacodynamics

The combined drug has an anti-inflammatory, antipruritic, antiallergic, local anesthetic effect.

Fluocortolone

The drug contains two forms of fluocortolone, which ensures a rapid onset of action and a longer duration. Fluocortolone, when exposed locally, prevents the marginal accumulation of neutrophils, which leads to a decrease in inflammatory exudate and the production of lymphokines, inhibition of macrophage migration, and a decrease in the processes of infiltration and granulation.

Cinchocaine

Cinchocaine is a local anesthetic. Pain relief is achieved by suppressing the formation and conduction of nerve impulses along afferent nerve fibers by depolarizing sodium channels.

Pharmacokinetics

Fluocortolone is hydrolyzed by esterases, enzymes at the site of inflammation to form fluocortolone, 11-ketofluocortolone and trimethylacetic acid. T1/2 from plasma of fluocortolone and its metabolites after rectal administration is approximately 1.3 and 4 hours, respectively.

Fluocortolone is excreted from the body in the form of metabolites mainly by the kidneys.

Use during pregnancy and breastfeeding

A number of epidemiological studies suggest a possible increased risk of developing cleft palate in newborns whose mothers received oral corticosteroids in the first trimester of pregnancy. There is insufficient data on the use of local corticosteroids during pregnancy, but in this case the likelihood of adverse effects is very low due to the minimal bioavailability of corticosteroids when applied topically.

Ultraproct ointment and suppositories should be prescribed with caution to pregnant women in the 2nd and 3rd trimester. When prescribing to pregnant and lactating women, it is necessary to compare the expected benefits of treatment for the mother with the possible risk to the fetus and infant.

Proctosan contains the anti-inflammatory substance bufexamac and bismuth subgallate, which has a regenerating and drying effect. Indications for the use of suppositories include:

  • anal fissures;
  • proctitis;
  • inflammatory processes in the rectal area;
  • anal eczema in acute and chronic form;
  • haemorrhoids.

Proctosan is considered one of the most effective medicines in proctology, as it quickly relieves unpleasant symptoms and promotes the healing of affected tissues. Unlike Ultraproct, which is not recommended for use for more than 4 weeks, it rarely causes side effects with prolonged use, and can be used as a prophylactic agent. The disadvantage of the drug is its high price.

Belladonna extract

Suppositories containing belladonna extract are one of the cheapest and most effective remedies against hemorrhoids. If you compare the medicine with Ultraproct, you can note the minimum number of contraindications - the ban exists only for treatment during pregnancy and lactation.

The effect is observed within a few minutes after use - pain and manifestations of the inflammatory process are significantly reduced.

Despite the fact that belladonna extract is a natural substance, it requires caution during treatment and compliance with medical recommendations.

The disadvantages include side effects from the use of the drug - many patients report indigestion, diarrhea and flatulence, and a burning sensation and discomfort are felt in the anal area. When using Ultraproct, such manifestations are observed much less frequently, and the hormonal components that are included in its composition act faster and more effectively.

Procto-Glyvenol

Procto-Glivenol suppositories are a combination drug that contains tribenoside and lidocaine. The first component refers to substances that have the ability to reduce vascular permeability and improve their tone, and lidocaine is a local anesthetic that reduces pain and itching. The main indication for the use of Procto-Glivenol is hemorrhoids, and the drug is effective even in advanced stages of the disease, when surgical treatment is impossible for some reason.

Ultraproct has a wider range of applications, but it has more contraindications and side effects due to the content of glucocorticosteroids. The cost of Procto-Glivenol is much lower, so it is often chosen as an effective and inexpensive substitute.

Natalsid

The basis of the drug is sodium alginate, which is obtained from brown algae, and solid fat (supposir, vitespol) acts as an additional component. Natalsid has anti-inflammatory, hemostatic and regenerative effects, quickly eliminates the symptoms of proctological diseases and improves general condition. Rectal suppositories are used for the following pathologies:

  • anal fissures;
  • hemorrhoids, accompanied by prolapse of nodes, bleeding, itching and pain;
  • inflammatory processes in the anal area;
  • proctosigmoiditis.

Natalsid has a minimal number of contraindications - these include age under 14 years and individual intolerance to the components of the drug. It also causes side effects extremely rarely - sometimes allergic manifestations, itching, burning and discomfort are possible.

Ultraproct more often causes unwanted reactions and has a wider list of contraindications, but the effect of using suppositories lasts much longer and its cost is lower. Accordingly, in case of severe symptoms, it is better to use Ultraproct, and for the treatment of children and pregnant women it is recommended to choose Natalsid as a safer natural remedy.

Anestezol

Rectal suppositories Anestezol are a combined drug of natural origin that contains four active ingredients:

  • benzocaine;
  • bismuth subgalate;
  • zinc oxide;
  • menthol.

It is intended to eliminate discomfort and improve the condition of patients with hemorrhoids and anal fissures. Anestezol has an anti-inflammatory, antiseptic, analgesic and cooling effect, accelerates tissue regeneration and facilitates bowel movements when bowel function is disrupted.

Compared to Ultraproct, Anestezol has a limited list of indications and a less pronounced therapeutic effect - the former contains glucocorticosteroids, which are considered the most effective means for stopping inflammatory processes.

Anestezol, in turn, is considered a safer drug and, if indicated, can be used during pregnancy. However, it should be borne in mind that with prolonged use it can cause dangerous disorders of the hematopoietic system, so it must be used with caution.

Directions for use and doses

Ultraproct is recommended for use after defecation. Before using the drug, you should perform hygiene of the anus (anus).

Rectal ointment

Typically, the ointment should be applied 2 times a day, on the first day for rapid relief of symptoms - up to 4 times.

Squeeze a small amount of ointment (about the size of a pea) onto your finger and lubricate the area around the anus and inside the anal ring. To overcome sphincter resistance, you need to apply the ointment with your fingertip. Then, pressing lightly on the tube, squeeze a small amount of ointment into the rectum.

Rectal suppositories

Typically, you should use 1 supp daily, injecting it deep into the rectum. In severe cases, on the first day 1 supp. is administered 2 or 3 times.

For severely inflamed and therefore painful hemorrhoids, it is recommended to start treatment with ointment.

Protruding hemorrhoids should be generously lubricated with ointment, gently pushing them back with your finger.

If the suppositories have softened under the influence of heat, they should be placed in cold water without removing the packaging, then they will return to their previous shape.

There is usually rapid improvement, but this should not be a reason to stop treatment too soon. To avoid relapses, you should continue to use Ultraproct for at least 1 more week, although with less frequency (ointment - 1 time per day or 1 supp. - every other day), even with complete disappearance of symptoms. However, the duration of treatment should not exceed 4 weeks.

How to choose the best analogue?

You need to choose the optimal remedy for the treatment of hemorrhoids after carefully studying the instructions for use, contraindications and side effects. In the presence of concomitant pathologies or in advanced stages of proctological diseases, the selection of the drug should be carried out only after consultation with a doctor.

Overdose

In accordance with the results of studies of the acute toxicity of fluocortolone and cinchocaine esters, acute intoxication is unlikely to occur with a single rectal or perianal use of Ultraproct, even with an accidental overdose. In case of accidental oral administration of the drug (for example, when swallowing several grams of ointment or several suppositories), it is likely that the side effects of cinchocaine are mainly of a somatic nature, which, depending on the dose, can manifest as severe cardiovascular symptoms (decreased cardiac activity up to cardiac arrest) , and symptoms of dysfunction of the central nervous system (convulsions, inhibition of respiratory function up to respiratory arrest).

Possible side effects and contraindications

There are quite a few contraindications to the use of the ointment, as indicated in the instructions for use. Analogues of Ultraprokt often have a less impressive list of contraindications, since the composition is slightly different. So, Ultraproct ointment is prohibited from being used for the following conditions and pathologies:

  • tuberculosis and syphilis in the area of ​​application of the drug;
  • pregnancy (in particular the first trimester);
  • diseases of viral etiology (chickenpox, immune response to recent vaccination);
  • hypersensitivity to fluocortolone or other main components of the drug.

The most common side effect from using the ointment is an allergic reaction in the area of ​​application, according to the instructions for use. Ultraprokt analogues can also provoke a number of allergic reactions. These are urticaria, swelling, skin atrophy, hypopigmentation, itching in the area of ​​application. If the patient notices the described side effects, then you should stop using Ultraproct ointment. Analogs with local anesthetic and anti-inflammatory effects can be no less effective in combating the disease. However, they will not cause side effects, since the composition will be different.
