Fruit composition

Popularity of Serenoa Repens extracts for the treatment of LUTS. associated with BPH has increased significantly in Europe and the USA [1,2,3]
A number of factors contributed to this:
- Significant deterioration in the quality of life of patients due to side effects from taking synthetic drugs (vertigo, ejaculation disorders, decreased libido, impotence, depression, decreased ejaculate volume, etc.) (13)

Extracts of Serenoa repens (creeping palm) have been well studied and have an extensive evidence base for their effectiveness and tolerance. At the same time, there are known studies that found no differences between the effectiveness of extracts and placebo. In practice, reviews about the effectiveness of these extractors sometimes differ polarly. The reasons for these contradictions are revealed in a number of fundamental works (14,15,16). In simple words, the effectiveness depends on the concentration of the extracts, i.e. on their content of fatty acids and sterols.
This obvious conclusion has been accepted and legislated in most countries:
In the USA, the quality and composition of Standardized Extracts is regulated by the American Pharmacopoeia USP–NF 36 as follows:
- Fatty acids – at least 80%
- Phytosterols – not less than 0.2%
- β-sitosterols – not less than 0.1%
In addition, in the USA there is a state standard SRM 3251 regulating the composition and quality control methods of Saw Palmetto extract, developed by the National Institute of Standards and Technology (NIST).
Both of these documents (Pharmacopoeia and Standard SRM 3251) define the content of Lauric acid as a marker of the authenticity of the extract, and the content of other fatty acids is determined by the ratio with the amount of lauric acid. The Standards Institute (US NIST) provides reference samples of the extract obtained by supercritical CO2 extraction for quality control and certification of extracts by drug manufacturers.
Extracts that meet these requirements may be labeled as “Standardized” at 85% or 45% fatty acids and sterols.
In Europe, the quality and composition of extracts is regulated by the “Committee on Herbal Medicines”.
The composition and quality requirements for Serenoa Repens extract are established by the European Pharmacopoeia 2014 as follows:
- Fatty acid content (total) – more than 80%
- including Laurinova - more than 23%
- Phytosterol content (total) – more than 0.2%
- including β-sitosterol – more than 0.1%
- As an additional quality parameter, it is allowed to indicate the ratio of the weight of the original plant material to the weight of the finished extract 6-12:1 (DER-Drug extract ratio)
As we see in Europe, as in the USA, the minimum content of lauric fatty acid in the extract is regulated.
The mentioned regulatory documents have made it possible to reduce the number of low-quality extracts in the US and European markets, and give an additional impetus to increasing confidence in Serenoa Repens extracts.
The absence in Russia of similar systems for certification, classification and quality control of Serenoa Repens extracts contributes to the filling of the Russian market with extracts of low quality, and often outright falsified. A sure sign of low quality, in most cases, is the lack of information about the concentration of active substances on the drug label and its substitution with DER [7].
In this case, the dosage is 320 or (160) mg. does not provide any information about the actual amount of active substances (fatty acids and sterols) and the therapeutic effectiveness of the drug.
In Russia, the use of herbal preparations is enshrined in the clinical recommendations “Benign Prostatic Hyperplasia 2019”, with a fair reservation about their weak therapeutic effectiveness, apparently referring to the herbal preparations prevailing on the Russian market.
Taking into account the above factors, in Russia the number of prescriptions of herbal preparations remains at a very low level, due to the prevailing fair-skeptical attitude of urologists and patients towards the effectiveness of most drugs available on the market containing Serenoa Repens fruit extract.

The main purpose of drugs based on saw palmetto is the treatment of prostatic hyperplasia, the growth of which is caused by the action of the most active androgen - dihydrotestosterone, formed from free testosterone under the influence of the enzyme 5α-reductase in the prostate gland. In addition, the extract has an anti-inflammatory effect, reduces vascular permeability and swelling of glandular tissue [Wilt].
Dihydrotestosterone is responsible for hair loss on the scalp and the growth of unwanted body hair in women. The difference is due to the type of 5α-reductase enzyme: thus, prostatic hyperplasia and scalp hair loss in men are often caused by excessive activity of the 5ap type 2 enzyme, while body hair growth in women is usually associated with the activity of 5ap type 1. The activity of certain enzymes in the body of different people is different, but the dwarf palm has shown that it can reduce the activity of both isoenzymes [Suzuki].

Impact on the female body

Saw palmetto preparations are sold in the form of extracts and powdered berries. The type of extraction affects the composition of the resulting drug: when extracting with liquid carbon dioxide, the result contains fatty acids and their ethyl and methyl esters, β-sitosterol, β-sitosterol 3-O-β-glycoside, campesterol, stigmasterol, lupeol, cycloartenol, 24- methylene cycloartenol, carotenoids; flavonoids, isoquercetin, kaempferol 3-O-glycoside, apigenin 7-O-rhamnoglycoside were found in the alcohol extract; in aqueous extract - polysaccharide fractions (galactose, arabinose, xylose, mannose, rhamnose, glucose and glucuronic acid) with anti-inflammatory and immunomodulatory effects.
Options for dietary supplements and pharmaceutical drugs
Many prescription drugs have been created based on serona, registered in Europe and Russia.
But in the USA, the FDA does not classify serona as a drug, so its sale is permitted in the form of dietary supplements. I purchased serona from the American electronic pharmacy iherb, and took it for quite a long time (6 months with two-week breaks + several more courses of 2 weeks each) + prescribed it for my father. In Russia it was twice as expensive: for example, the minimum dose of 320 mg/day when purchased from iherb will now cost 12 rubles, and in Russia - 20-25 rubles.
